Lymphangitis carcinomatosa in pleural mesothelioma

Abstract

A male, 65-year-old patient investigated because of right pleural effusion. Asbestos exposure was not certain. On his physical examination, we found decreased volume of right hemithorax and signs associated with right pleural fluid. The malignant mesothelioma diagnosis was obtained at last by decortication. Two mounts later from diagnosis, the patient admitted to intensive care unit with respiratory failure accompanied by infection signs. The additional findings were severe dyspnea and fever, rales over left lung, the roentgenographic changes as diffuse reticulonodular pattern on left lung. The postmortem left lung biopsy showed lymphangitis carsinomatosa. We found only one peritoneal, two pleural mesothelioma case with lymphangitis carcinomatosa in the literature. As a rare type of spread, we approved to report that case especially to emphasize the importance of palliative approach.

Özet

Altmışbeş yaşında erkek hasta sağ plevral sıvı nedeniyle tetkike alındı. Asbestoz teması kuşkuluydu. Muayenesinde sağ omuz ve göğüste çöküklük ile sağ plörezi bulguları saptandı. Sonuçta dekortikasyonla malign mezotelyoma tanısı aldı. İki ay içinde infeksiyon bulgularının da eşlik ettiği solunum yetmezliği tablosu ile yoğun bakıma yatırılan hastada ağır dispne, ateş yüksekliği, sol akciğerde inspiratuvar raller, akciğer röntgenogramında solda diffüz retikülonodüler patern saptandı. Postmortem sol akciğer açık biyopsisi lenfanjitis karsinomatoza ile uyumluydu. Literatür taramasında bu yayılımı göstermiş bir peritoneal, iki plevral mezotelyoma olgusuna rastladık. Mezotelyomada ender bir yaydım şekli olarak olgumuzu, palyatif tedavi yaklaşımlarında lenfanjitis karsinomatozanın yer alması için yayınlamayı uygun bulduk.
