The analysis of the correlation between atopic and nonatopic bronchial asthma and HLA-class II molecules

Abstract
Asthma is a syndrome characterized by airflow obstruction, inflammation and bronchial hyperresponsiveness and begins with the presentation of antijen to T cell by major histocompatibility complex class II molecule of antigen presenting cell (APC). MHC class II molecules and specific peptides of antigen form a complex which provides the recognition of antigen with T cells. There is a growing number of studies investigating the association of HLA antigens with asthma in different population and with specific antigens causing to atopy. In order to evaluate the association between Bronchial Asthma and HLA gen products in Turkish population, we aim to investigate HLA-DR and HLA-DQ antigen frequencies in 30 atopic, 20 nonatopic asthmatics and 50 control subjects. HLA typing for DR and DQ loci was done on isolated peripheral blood lymphocytes by standart Terasaki microcytotoxicity assay. Frequency of HLA antigen occurence was compared between control subjects and atopic, nonatopic asthmatics using Chi-Square test. Fisher and Yates exact correction tests used to detect significange of Relative Risk (RR). We observed significant differences in the frequencies of HLA-DRW6, HLA-DRW17 (3), HLA-DQW1 antigens in atopic and non atopic asthmatics and control subjects. HLA-DRW6 antigen was not observed either atopic or nonatopic asthmatics. HLA-DRW17 (3) antigen frequency was significantly higher in nonatopic asthmatics compared to that in control subjects. We found a negative association between HLA DQW1 antigen and atopic asthma with a corresponding decrease in RR= 0.35

Bronşiyal astmada inflamasyon, bronşiyal hiperreaktivite ve bronkospazm ile sonuçlanacak olaylar zinciri, antijenin antijen sunucu hücreler ve MHC-Klas II gen peptitlerince T lenfositlere sunumu ile başlar. MHC-Klas II gen peptitleri antijen fragmanlarını işleyerek CD4+T lenfositler tarafından tanınmasını sağlar. Bu immünopatolojik mekanizmadan yola çıkarak Tıp Fakültemiz Göğüs Hastalıkları ve Tüberküloz Anabilim Dalı Astma Polikliniği'ne başvuran hastalardan 20 atopik , 30 nonatopik toplam 50 bronşiyal astmalı olguda, ortak bir HLA-DR ve HLA-DQ doku tipi antijeninin varlığını araştırmak amacıyla çalışma gerçekleştirildi. HLA DR ve DQ antijenlerinin tiplendirilmesi standart Terasaki mikrositotoksisite yöntemi ile periferik kan lenfositlerinde yapıldı. HLA antijenlerinin sıklığının karşılaştırılması atopik, nonatopik astmalılar ve kontrol grubu arasında Chi-square istatistik analiz yöntemi kullanılarak yapıldı. Antijenleri taşıyanlar için hesaplanan rölatif riskleri anlamlılığı Fisher ve Yates istatistik test yöntemleri ve kontrol edildi. Araştırmamız sonucunda atopik ve nonatopik astmalılar ve kontrol grubu arasında HLA-DRW6, HLA-DRW17 (3) ve HLA-DQW1 antijenlerinin sıklıkları arasında anlamlı farklılıklar saptandı. HLA-DRW6 antijeni atopik ve nonatopik astmalılar arasında saptanmadı. HLA-DRW17 (3) antijeni sıklığı nonatopik astmalılarda kontrol grubuna göre anlamlı derecede yüksekti. HLA-DQW1 antijenin sıklığı ile atopik astma arasında negatif bir korelasyon gözlemlememiz nedeniyle, bu antijeni taşıyan kişilerde atopik astma gelişme riskinin (RR=0.35) azaldığı sonucuna varıldı.
