The use of prophylactic antibiotics in lung cancer surgery

Abstract

Antibiotic prophylaxis in pulmonary resections for bronchogenic carcinoma has become routine practice in most of the thoracic surgery departments. But the duration of prophylaxis remains controversial. This study was planned to determine the clinical efficiency of sefepime prophylaxis applied in three different regimens. One hundred and twelve patients who underwent pulmonary resections for bronchogenic carcinoma were divided into three groups: Group 1 patients (n= 54) were given cefepime intravenously every 12 hour for 48 hours, group 2 patients (n= 34) were given cefepime intravenously every 12 hour for five days and group 3 patients (n= 24) were given a single-dose cefepime intravenously at the time of the anesthetic induction. Any evidence for infection were recorded in each group postoperatively. Infection findings were noted in six patients in group 1, four in group 2 and nine in group 3. Statistically significant differences were seen in between group 1 and 3, and between group 2 and 3 (p< 0.05 for both). Meanwhile, comparison of the groups 1 and 2 revealed statistically insignificant difference (p= 0.59). As a result, 48-hour or five-day antibiotic prophylaxis regimen with cefepime were found to be more efficient than a single-dose prophylaxis in pulmonary resections. In addition, a 48-hour administrationn suffices, and long term administration fails to offer any further advantages.

Özet

Bronş karsinomu nedeniyle yapılan akciğer rezeksiyonlarında antibiyotik profilaksisi, çoğu göğüs cerrahisi kliniğinde rutin hale gelmiştir. Ancak uygulamanın ne kadar sürdürüleceği konusu tartışmalıdır. Bu çalışma, üç farklı şekilde uygulanan sefepim profilaksisinin klinik etkinliğini araştırmak üzere planlandı. Primer bronş kanseri tanısı ile akciğer rezeksiyonu uygulanan 112 hasta çalışmaya alınarak 3 gruba ayrıldı: 1. gruba dahil edilen 54 hastaya 48 saat süreyle 12 saatte bir intravenöz sefepim, 2. gruptaki 34 hastaya 5 gün süreyle 12 saatte bir intravenöz sefepim verildi; 3. grupta yer alan 24 hastaya ise sadece anestezi indüksiyonu sırasında tek doz intravenöz sefepim uygulandı. Postoperatif dönemde her grup içinde infeksiyon delili saptanan hastalar kaydedildi ve istatistiksel olarak incelendi. Birinci grupta altı, 2. grupta dört ve 3. grupta ise dokuz hastada infeksiyon bulgusu gözlendi. Birinci grup ile 3. grup arasında, 2. grup ile 3. grup arasında istatistiksel olarak anlamlı fark izlendi (her ikisi için p< 0.05). Diğer yandan 1. grup ve 2. grup kıyaslandığında, aralarındaki fark istatistiksel olarak anlamsızdı (p= 0.59). Sonuç olarak, akciğer rezeksiyonlarında, sefepim ile 48 saat ya da 5 gün uygulanan antibiyotik profilaksisinin, tek doz sefepim ile yapılan profilaksiye göre daha etkili olduğu görüldü. Ayrıca, 48 saat uygulamanın yeterli olduğu, profilaksiyi daha uzun sürdürmenin ilave yarar sağlamadığı sonucuna varıldı.
