The efficacy and utilisation of preoperative multiparametric magnetic resonance imaging in robot-assisted radical prostatectomy: does it change the surgical dissection plan?

Abstract
Objective: We investigated the effect of the use of multiparametric prostate magnetic resonance imaging(mp-MRI) on the dissection plan of the neurovascular bundle and the oncological results of our patients whounderwent robot-assisted radical prostatectomy.Material and methods: We prospectively evaluated 60 consecutive patients, including 30 patients who had(Group 1), and 30 patients who had not (Group 2) mp-MRI before robot-assisted radical prostatectomy. Basedon the findings of mp-MRI, the dissection plan was changed as intrafascial, interfascial, and extrafascial inthe mp-MRI group. Two groups were compared in terms of age, prostate-specific antigen (PSA), Gleason sumscores and surgical margin positivity.Results: There was no statistically significant difference between the two groups in terms of age, PSA, biopsyGleason score, final pathological Gleason score and surgical margin positivity. mp-MRI changed the initial surgicalplan in 18 of 30 patients (60%) in Group 1. In seventeen of these patients (56%) surgical plan was changedfrom non-nerve sparing to interfascial nerve sparing plan. In one patient dissection plan was changed to non-nervesparing technique which had extraprostatic extension on final pathology. Surgical margin positivity was similar inGroups 1, and 2 (16% and 13%, respectively) although, Group 1 had higher number of high- risk patients. mp-MRIconfirmed the primary tumour localisation in the final pathology in 27 of of 30 patients (90%).Conclusion: Preoperative mp-MRI effected the decision to perform a nerve-sparing technique in 56% of thepatients in our study; moreover, changing the dissection plan from non-nerve-sparing technique to a nervesparing technique did not increase the rate of surgical margin positivity.

Amaç: Çalışmamızda multiparametrik prostat manyetik rezonans görüntüleme (mp-MRG) kullanımının robot yardımlı radikal prostatektomi hastalarında nörovasküler demet diseksiyon planına ve onkolojik sonuçlara etkisi araştırıldı. Gereç ve yöntemler: Çalışmamıza 30’u operasyon öncesi mp-MRG yapılan 30’u ise mp-MRG yapılmayan toplam 60 robot yardımlı radikal prostatektomi adayı hasta alındı. mp-MRG yapılan grupta bulgulara göre nörovasküler diseksiyon planı intrafasyal, interfasyal ve ekstrafasyal olarak belirlendi. İki grup yaş, prostat spesifik antijen (PSA), Gleason skorları ve cerrahi sınırı pozitifliği açısından karşılaştırıldı. Bulgular: İki grup arasında yaş, PSA düzeyi, biyopsi Gleason skoru, prostatektomi Gleason skoru ve cerrahi sınır pozitifliği açısından istatistiksel fark saptanmadı. mp-MRG başlangıç nörovasküler diseksiyon planını 30 hastanın 18’inde (%60) değiştirdiği görüldü. On yedi hastada (%56) cerrahi diseksiyon planı interfasyal sinir koruyucu diseksiyon olarak değiştirilirken 1 hastada sinir korunmama kararı verildi. mp-MRG yapılan grupta daha fazla sayıda yüksek riskli hasta bulunmasına rağmen; mp-MRG yapılan ve yapılmayan hastalarda cerrahi sınır pozitiflikleri sırasıyla %16 ve %13 olarak birbirine yakın saptandı. mp-MRG’nin primer tümörü 30 hastanın 27’sinde (%90) belirlediği prostatektomi patolojisi ile gösterildi. Sonuç: Çalışmamızda operasyon öncesi mp-MRG’nin %56 hastada sinir koruyucu diseksiyon yapılmasını sağladığı gösterildi. Bununla beraber mp-MRG ile saptanan sinir koruyucu diseksiyonun cerrrahi sınır pozitifliğini artırmadığı saptandı.
