An update of Penile Fractures: Long-term significance of the number of hours elapsed till surgical repair on long-term outcomes

Abstract

Objective: We updated our data on penile fractures and investigated the significance of the time intervalfrom the incident of the fracture until the operation on the erectile functions and long-term complications.Material and methods: Between January 2001 and June 2014, 64 patients were operated on with a preoperativediagnosis of penile fracture. We could evaluate 54 of these patients. The patients were classifiedinto 3 groups according to the time interval from the time of fracture until surgery. The validated Turkishversion of the erectile components of International Index of Erectile Function (IIEF) was answered by everypatient 3 times after the surgery; before the incident of fracture, at first postoperative year, and at the time ofthe study (IIEF-5 and question #15 were used). The complications were noted and an erectile function indexscore was calculated for every patient.Results: Mean follow up period was 44.9 (±2.8) months for all patients There was no statistically significantdifference between the 3 groups in terms of the erectile components of IIEF questionnaire scores forthe time periods and for individual patients in each separate group. Complications for all groups were alsosimilar.Conclusion: In consideration of long-term results, neither serious deformities nor erectile dysfunction occuras a consequence of a delay in surgery performed within the first 24 hours in patients without urethralinvolvement.

Özet

Amaç: Kliniğimizde penil fraktürler ile ilgili birikimimizi güncelleyerek fraktür ile cerrahi tedavi arasında geçen sürenin uzun dönemde erektil fonksiyonlar ve komplikasyonlar üzerine etkisini inceledik. Gereç ve yöntemler: Ocak 2001 ile Haziran 2014 tarihleri arasında 64 hasta preoperatif penil fraktür tanısı ile opere edildi. Bunlardan 54 tanesi değerlendirmeye alınabildi. Fraktür ile cerrahi arasında geçen zaman dilimine göre hastalar 3 gruba ayrıldı. Uluslar arası erektil fonksiyonlar indeksinin (International Index of Erectile Function) valide edilmiş Türkçe versiyonun 6 sorusu her hasta tarafından; fraktür öncesi dönem için, operasyondan 1 yıl sonrası için ve çalışma esnasındaki güncel dönem için olmak üzere, cerrahi sonrasında 3’er defa dolduruldu (IIEF-5 ve 15. soru kullanıldı). Komplikasyonlar kaydedildi ve her hasta için erektil fonksiyonlar indeksi hesaplandı. Bulgular: Ortalama takip süresi tüm hastalar için 44,9 (±2,8) ay idi. Her 3 grup için ve grup içi bireysel olarak hastalar arasında IIEF sorgulamaları açısından anlamlı bir istatistiksel fark yoktu. Komplikasyonlar her 3 grup için benzerdi. Sonuç: Uzun dönem sonuçlara bakıldığında; gerek erektil fonksiyonlar açısından, gerekse komplikasyonlar açısından, ilk 24 saat içerisinde ve üretra hasarı olmamak kaydıyla, cerrahi tedaviye gidişteki bir gecikme hastalarda olumsuz bir sonuç doğurmamaktadır
