Abstract

Obstructive sleep apnea (OSA) is an important healthproblem that is increasing with the increase in the incidence ofobesity. For patients with OSA, it is reported that preoperativeOSA diagnosis is made in only 10%-20% of the patients. Therefore,it gains importance that these patients are diagnosed in thepreoperative period. The aim of this study is to analyze with asurvey the experiences of anaesthesia experts working in Turkeyabout the diagnosis of patients with OSA, anaesthetic methodsand postoperative care.Methods: The study was completed with an online survey formdistributed via email. Survey participants were sent an email, andthose experts who wished to participate clicked on the link, answeredthe questions and returned the survey via email.Results: A total of 134 anaesthesia experts participated in thisstudy. While 97% of the participating anaesthetists considered diagnosisof patients with OSA to be important, only 53% trustedthemselves to recognize this patient group. Of the total numberof participants. 43% did not know the STOP-BANG test usedin the preoperative period to diagnose patients with OSA. Thepercentage who thought that patients with OSA would increasethe incidence of difficult intubation was 84%. In the intraoperativeperiod, the percentage who chose inhalation anaesthetics was35%, whereas the rate of those who chose total intravenous anesthesia(TIVA) was 48%.Conclusion: This study indicates that the medical knowledge ofTurkish anaesthetists on the topic of OSA requires updating. Inaddition, we believe a practical guide to standardize the diagnosisand preoperative/perioperative management of patients with OSAshould be prepared for Turkish anaesthetists.

Obstrüktif uyku apnesi (OSA) obezite sıklığının da artmasıylabirlikte gittikçe büyüyen önemli bir sağlık sorunudur.Ancak, OSA'lı hastaların preoperatif olarak yalnızca %10-20'sinetanı konduğu belirlenmiştir. Bu nedenle bu hastaların preoperatifdönemde tanınması önem kazanmaktadır Çalışmamızın amacı,Türkiye'de çalışmakta olan anestezi uzmanlarının OSA'lı hastalarıntanınması ve anestezi yönetimleri ve postoperatif bakımları ileilgili tecrübelerinin bir anket ile analiz edilmesidir.Yöntemler: Araştırma anket formatında olup internet ortamındae-mail yoluyla gerçekleştirildi. Anket katılımcılara e-mail yoluylagönderildi ve ankete katılmak isteyen uzman hekim kendisinegelen linki tıklayarak soruları cevaplayıp yine e-mail yoluyla gönderdi.Bulgular: Bu çalışmaya toplam 134 anestezi uzmanı katıldı. Katı-lımcıların %97'si OSA'lı hastaların tanınmasını önemli olduğunudüşünmekte ancak bunların sadece %53'ü bu hastaları tanımadakendine güvenmektedir. Katılımcıların %43'ü OSA hastalarınınpreoperatif dönemde tanımada kullanılan STOP-BANG skorunubilmemekte. OSA'lı hastalarda zor entübasyon olasılığının arttı-ğını düşünenlerin oranı ise %84 olarak bulunmuştur. Bu hastagrubunda intraoperatif dönemde inhalasyon anestezisini tercihedenlerin oranı %35, TIVA tercih edenlerin oranı ise %48'dir.Sonuç: Bu çalışma anestezi uzmanlarının OSA konusunda tıbbibilgilerini güncellemesi gerektiğini göstermiştir. Ayrıca Türkiye'deanestezistler için OSA'lı hastaları tanınması ve perioperatif/postoperatifyönetiminde standardizasyonu sağlanması için pratik birkılavuz hazırlanması gerektiğini düşünüyoruz.
