Radiographic analysis of the prevalence of taurodontism in molar teeth in a sample from the Turkish population

Abstract
Background: Taurodontism is a developmental anomaly characterized by the anatomical structure of the tooth. Taurodontism, which is one of the most important of the structural anomalies of the tooth, is manifested by elongated pulp chamber, displaced pulp floor towards the apical, and divided into two or three short root/ canal structures on the radiograph. Taurodont teeth have abnormal root canals due to the number, shape and complex anatomy of the roots. The aim of this study is to determine the prevalence of taurodontism and to examine its localization, distribution and gender differences among patients who applied to the Faculty of Dentistry of Kutahya Health Sciences University. Methods: In this study, digital panoramic radiographs of 500 patients who applied to the Kutahya Health Sciences University Faculty of Dentistry Department of Endodontics between March 2016 to March 2018 due to various dental problems were analyzed retrospectively in terms of taurodontism. The frequency of the types of taurodontism and their incidence in terms of gender and jaws were evaluated. Statistical analysis of the data obtained was done by Pearson chi-square test. Results: A total of 500 panoramic radiographs (279 men, 221 women) were evaluated in the study. Taurodontism anomaly was detected in 37 (7.4%) of these radiographs and 56 (1.4%) of 4126 molar teeth examined. 26 (46.4%) of these teeth were classified as hypotaurodont, 10 (17.9%) were mesotaurodont, 7 (12.5%) were hyperturodont and 13 (23.2%) were pyramidal teeth. It was observed that 37 (66.1%) of the taurodont teeth were in the mandible and 19 (33.9%) were in the maxilla. It was found that the prevalence of taurodontism was higher in the mandible than the maxilla (p < 0.05). The prevalence of taurodontism was higher in females than in males (p < 0.05). Conclusion: Taurodontism is a common clinical anomaly. While dentists are planning endodontic treatment for molar teeth, they should be careful in terms of taurodontism anomaly that can be observed in these teeth

Amaç: Taurodontizm, dişin anatomik yapısıyla karakterize gelişimsel bir anomalidir. Dişin yapısal anomalilerinin en önemlilerinden biri olan taurodontizm, radyografide vertikal olarak uzamış pulpa odası, apikale doğru yer değiştirmiş pulpa tabanı ve iki ya da üçe ayrılmış normale göre kısa kök/kanal yapıları ile kendini göstermektedir. Taurodont dişler köklerin sayısı, şekli ve karmaşık anatomisi nedeniyle anormal kök kanallarına sahiptir. Bu çalışmanın amacı Kütahya Sağlık Bilimleri Üniversitesi Diş Hekimliği Fakültesi Endodonti Anabilim Dalı’na başvuran hastalar arasında taurodontizm prevalansını belirlemek, lokalizasyonu, dağılımını, cinsiyetler arasındaki farklılığını incelemektir. Gereç ve Yöntemler: Bu çalışmada çeşitli dental problemler nedeniyle Kütahya Sağlık Bilimleri Üniversitesi Diş Hekimliği Fakültesi Endodonti Anabilim Dalı’na Mart 2016 – Mart 2018 tarihleri arasında başvuran 500 hastanın dijital panoramik radyografları taurodontizm açısından retrospektif olarak incelendi. Taurodontizm tiplerinin görülme sıklığı ile cinsiyet ve çeneler açısından görülme oranları değerlendirildi. Elde edilen verilerin istatistiksel analizi Pearson ki-kare testi ile yapıldı. Bulgular: Çalışmada toplam 500 adet panoramik radyografi (279 erkek, 221 kadın) değerlendirildi. Bu radyografların 37 (% 7.4)’sinde ve incelenen 4126 molar dişin 56 (% 1.4)’sında taurodontizm anomalisi saptandı. Bu dişlerin 26 (% 46.4) tanesi hipotaurodont, 10 (% 17.9) tanesi mezotaurodont, 7 (% 12.5) tanesi hipertaurodont ve 13 (% 23.2) tanesi piramidal diş olarak sınıflandırıldı. Taurodont dişlerin 37 (% 66.1)’sinin mandibulada, 19 (% 33.9)’unun maksillada olduğu görüldü. Mandibulada taurodontizm prevalansının maksillaya oranla daha yüksek olduğu tespit edildi (p < 0.05). Kadınlarda taurodontizm prevalansı erkeklerden anlamlı derecede daha yüksek bulundu (p < 0.05). Sonuç: Taurodontizm yaygın olarak görülen klinik bir anomalidir. Hekimler molar dişlere endodontik tedavi planlarken, bu dişlerde görülebilecek taurodontizm anomalisi açısından dikkatli olmalıdır
