Association Between Titration Method and Outcomes of First Night Satisfaction and CPAP Compliance

Abstract
Introduction: The gold standard therapy for obstructive sleep apnoeasyndrome (OSAS) is continuous positive airway pressure (CPAP). Ourstudy evaluated the relationship between first night satisfaction andCPAP compliance, and the effect of using the titration method in thisrelationship.Methods: Between January 2012 and December 2012, 40 patients withthe diagnosis of OSAS and without comorbidities, who had undergoneCPAP titration, were included. Of these, 20 patients had undergonemanual titration (MT) with polysomnography, whereas 20 patients hadundergone auto-adjusted CPAP (APAP) titration. Questionnaires wereadministered during the morning of the titration night. The first-yearadherence to treatment and patient compliance were evaluated.Results: Forty patients were randomised in two groups; 35 patientswho could procure the CPAP and were available at the end of the firstyear completed the study. From the 35 patients, 20 (6 males; 40%) werein the MT group while 15 (6 males; 30%) were in the APAP group. Nosignificant difference was detected between the mean titration pressurelevels obtained with the MT or APAP group. There was no differencein terms of responses to the questions on the questionnaire betweenthe two groups. At the first-year evaluation, no significant difference wasdetected between the groups.Conclusions: The titration method used to detect CPAP pressure, MT,or APAP does not affect patient satisfaction on the day following thetitration night, and does not affect first-year compliance.

Amaç: Sürekli pozitif havayolu basıncı (CPAP), obstrüktif uyku apnesi sendromu (OSAS) için altın standart tedavidir. Çalışmamız, ilk gece memnuniyeti ile CPAP uyumluluğu arasındaki ilişkiyi ve bu ilişkide titrasyon yönteminin kullanılmasının etkisini değerlendirmiştir. Yöntem: Çalışmaya Ocak 2012–Aralık 2012 tarihleri arasında ek hastalığı bulunmayan OSAS tanısı ile CPAP titrasyonu uygulanan 40 hasta dahil edildi. Olguların 20’sinde polisomnografi (PSG) ile birlikte manuel titrasyon uygulanırken diğer 20’sinde otomatik CPAP titrasyonu (APAP) uygulandı. Titrasyon gecesi sonrası sabah ise memnuniyet anketleri uygulandı. Ayrıca tedavinin birinci senesinde tedaviye bağlılık ve hasta uyumluluğu değerlendirildi. Bulgular: Kırk hasta randomize olarak iki gruba ayrıldı; CPAP temin edebilen ve birinci yılın sonunda mevcut olan 35 hasta çalışma tamamladı. 35 hastadan 20’sinde (6 erkek; %40) manuel titrasyon (MT) grubunda; 15’i (6 erkek; %30) APAP grubundaydı. MT veya APAP grubuyla elde edilen ortalama titrasyon basınç seviyeleri arasında anlamlı bir fark tespit edilmedi. Her iki grup arasındaki anket sorularına verilen yanıtlar arasında bir farklılık yoktu. İlk yıl değerlendirmesinde, gruplar arasında anlamlı bir fark tespit edilmedi. Sonuç: CPAP basıncını saptamak için kullanılan titrasyon yönteminin MT veya APAP olması, titrasyon gecesini takip eden günde hasta memnuniyetini ve birinci yıl uyumu etkilememektedir.
