Surgical Management of Peripheral Artery Pathologies After Catheterization

Abstract

Objective: At last years, the frequency of diagnostic and therapeutic endovascular interventions has increased with the increasing num‐ ber of centers and developing technology. The femoral artery remains the most common vascular access for coronary and peripheral artery interventions. As any procedure, complications can occur. These are typically associated with inadequate puncture techniques. Among these complications, pseudoaneurysms have the highest incidence, others are organ ischemia and arteriovenous fistula forma‐ tion. Sixty‐two peripheral vascular pathology cases who underwent surgical intervention after endovascular catheterization in our clinic in the last six years were retrospectively investigated and the surgical treatment approaches applied were evaluated. Material and Method: The records of patients who developed peripheral artery pathology after cardiac catheterization in our clinic were analyzed retrospectively. The study period was determined between January 2015 and January 2021. During this period, 62 pa‐ tients with peripheral vascular pathology who underwent only surgical intervention after cardiac catheterization were detected. Cases with peripheral vascular pathology due to other reasons were not included in the study. The complaints of patients with vascular pathol‐ ogy occurred two to seven days after coronary angiography. The main complaints of the patients were pain, swelling, pulstatile mass and bruising in the application area. Physical examination and color Doppler ultrasonography were used primarily to detect the vascu‐ lar pathology of the patients and they were considered adequate. Results: The mean age of the patients was 64.8±8.98 (48‐84 years). Forty of the patients were male (n=40, 64.5%), 22 were female (n=22 35.5%). When the complaints of the patients were examined, pain at the intervention site (n=32, 51.6%), pulstatile mass (n=19 30.6%), extremity ischemia findings (n=11, 17.7%) were the most frequently reported complaints. Intervention sites used for catheteri‐ zation in patients are the right lower extremity (n=46, 74.2%), left lower extremity (n=8, 12.9%), left upper extremity (n=8, 12.9%), in order of frequency. Intervention from the upper extremity was not detected in the investigated patient group. Results: Despite the increasing popularity of other treatment methods, surgical intervention should be planned and applied in the pres‐ ence of a surgical intervention indication for peripheral vascular pathologies that develop after catheterization. We believe that early diagnosis and surgical intervention are important by preventing complications, and if reconstruction is necessary, arterial repair should be done as soon as possible.

Amaç: Son yıllarda tanısal ve tedavi amaçlı endovasküler girişim sıklığı artan merkez sayısı ve gelişen teknolojiyle artmaktadır. Femoral arter, koroner ve periferik arter girişimleri için en yaygın vasküler erişim olmaya devam etmektedir. Her prosedürde olduğu gibi komp‐ likasyonlar ortaya çıkabilir. Bunlar tipik olarak yetersiz ponksiyon teknikleri ile ilişkilidir. Bu komplikasyonlar içinde insidansı en yüksek olan psödoanevrizmalar olup, diğerleri organ iskemisi ve arteryovenöz fistül oluşmasıdır. Kliniğimizde son altı yıl içerisinde endovaskü‐ ler kateterizasyon sonrası cerrahi girişimde bulunulan 62 periferik vasküler patoloji vakası geriye dönük olarak araştırılıp, uygulanan cer‐ rahi tedavi yaklaşımları incelendi. Gereç ve Yöntem: Kliniğimizde kardiyak kateterizasyon sonrası periferik arter patoloji olgusu gelişen hastaların kayıtları geriye dönük olarak incelendi. Çalışma periyodu Ocak 2015 ile Ocak 2021 tarihleri arası olarak belirlendi. Bu dönemde sadece kardiyak kateterizas‐ yon sonrası cerrahi müdahale uygulanan 62 periferik vasküler patolojiye sahip olgular saptandı. Diğer sebeplerle oluşan periferik vas‐ küler patolojiye sahip olgular çalışmaya dahil edilmedi. Oluşan vasküler patolojiye sahip hastaların şikayetlerinin koroner anjiyografiden iki ile yedi gün sonrasında oluştuğu tespit edildi. Hastaların başlıca şikayetlerinin uygulama bölgesinde ağrı, şişlik, pulstatil kitle ve mo‐ rarma olduğu görüldü. Hastaların vasküler patolojisini tespit etmede fizik muayene ve renkli Doppler ultrasonografi primer olarak kul‐ lanılıp yeterli olarak kabul edildi. Bulgular: Hastaların yaş ortalaması 64,8±8,98 (48‐84 yıl) idi. Hastaların kırkı erkek (n=40, %64,5), yirmi ikisi kadındı (n=22, %35,5). Hasta şikayetleri incelendiğinde girişim yerinde ağrı (n=32, %51,6), pulstatil kitle (n=19, %30,6), ekstremite iskemi bulguları (n=11, %17,7) en çok bildirilen şikayetler olarak tespit edildi. Hastalarda kateterizasyon için kullanılan girişim yerleri ise sıklık sırasına göre sağ alt ekst re‐ mite (n=46, %74,2), sol alt ekstremite (n=8, %12,9), sol üst ekstremite (n=8, %12,9) olup sağ üst ektremiteden girişim araştırılan hasta grubunda tespit edilmemiştir. Sonuç: Popülaritesi artan diğer tedavi yöntemlerine rağmen kateterizasyon sonrası gelişen periferik vasküler patolojilerin cerrahi giri‐ şim endikasyonu varlığında vakit geçirilmeden yıllardır uygulanan etkinliği bilinen cerrahi müdahale planlanıp uygulanmalıdır. Komp li ‐ kas yonların önlenerek erken tanı ve cerrahi girişim yapılması önemli olup, rekonstrüksiyon zorunlu ise mümkün olan en kısa sürede ar‐ teryel onarımın yapılması gerektiğine inanmaktayız.
