Comparison of sodium phosphate and sennoside solutions in bowel preperation before colonoscopy: Prospective randomised trial

Abstract
Purpose: A good colon cleansing is necessary to perform an adequate and full colonoscopy. Some colonic lesions can be overlooked with an inadequate bowel preperation and reprepation causes an increased cost and time. Th e aim of this study is to compare the effi cacy, side eff ects and patients’ tolerability of sodium phosphate and sennoside solutions in precolonoscopic colon cleansing. Method: Sixty patients were included in the study. Th e patients were consecutively randomised into two groups. All patients received liquid diet for two days before the examination. In the fi rst group, the patients received 90 ml NaP (45 ml + 45 ml), and in the second groups patients received 500 ml sennoside (250 ml + 250 ml) at the day prior to colonoscopy. Before colonoscopy, patients were questioned about the complaints and were also asked if they woued prefer the samesolutions or not, if repreperation is needed. At the end of each colonoscopy, another colonoscopist blindly scored cleansing according to the scoring system (1 excellent -4 bad). Results compared with student-t test. Results: Th e male/female ratio in group 1 was 17/13 and the median age was 56 years, while the male/female ratio was16/14 and the median age was 59 years in group 2. Abdominal pain, vomiting and nausea were seen at three patients in sennoside group. No side eff ects were seen at NaP group. Twenty-two patients (73%) from group 1, twelve patients (40%) from group 2 responded that they could use the same solution for bowel preperation if recolonoscopy is needed. Th e median cleansing grade for the fi rst group was 1.47 0.63 and it was 1.61 0.76 for the second group (p=0.417). Conclusion: Both oral solutions after two days liquid diet proved to be equally eff ective and safe. However, patient tolerance of NaP demonstrated a clear advantage over sennoside.

Amaç: Kolonoskopinin uygun ve tam yapılabilmesi için iyi barsak temizliği şarttır. Yetersiz barsak temizliği, lezyonların atlanmasına sebep olabileceği gibi yeniden hazırlık yapılmasına da neden olarak maliyeti artıracaktır. Çalışmamızda barsak temizliği için kullanılan ilaçlardan, sodyum fosfat (NaP) ve sennosid’in barsak temizliğindeki etkinliğini, yan etkilerini ve hasta toleransını değerlendirmeyi amaçladık. Yöntem: Yaş-cinsiyet ayırımı yapmaksızın ardışık 60 hasta çalışmaya alındı. Hastalar rastgele 2 gruba ayrıldı. Tüm hastalara 2 günlük berrak diyet önerildi. Birinci gruba, kolonoskopiden 1 gün önce, 90ml NaP (45ml + 45ml) ile ikinci gruba ise 500 ml sennosid (250ml + 250ml) ile barsak temizliği yapıldı. Hastalar kolonoskopiden önce, bir daha kolonoskopi yapılması gerekirse aynı ilacı kullanmak isteyip-istemeyecekleri ve ilaç alımı sonrası gelişen yan etkiler açısından sorgulandı. Kolon temizliği, hangi ilaçla hazırlık yapıldığını bilmeyen bir başka araştırmacı tarafından 1’den 4’e kadar (1 mükemmel - 4 kötü) sınıfl andırıldı. Bulgular: Birinci gruptaki erkek/kadın oranı 17/13 olup, yaş ortalaması 56 idi. Aynı oranlar 2.grupta 16/14 ve 59 yaş idi. Sennosid grubunda 3 hastada ilaç alımı sonrasında abdominal kramp, mide bulantısı ve kusma oldu. NaP grubunda herhangi bir yan etki görülmedi. Birinci gruptaki hastaların 22 (%73)’si yeniden kolonoskopi yapılması gerekirse aynı ilacı almayı kabul ederken, bu oran 2. gruptaki hastalarda 12 (%40) idi. Birinci grupta kirlilik derecesi (1 mükemmel - 4 kötü) 1.47 ± 0.63, ikinci grupta ise 1.61 ± 0.76 olarak bulundu (p=0.417). Sonuç: Kolonoskopi öncesi iki gün boyunca berrak diyet ve sonrasında 90ml NaP veya 500 ml sennosid solüsyonu yeterli barsak temizliği sağlamaktadır. NaP, sennoside göre hastalar tarafından daha iyi tolere edilmektedir.
