Efficacy of peripheral neuromodulation in the treatment idiopatic slow transit constipation

Abstract

Purpose: Idiopathic slow transit constipation (ISTC) is the form of constipation in which the colonic and rectal morphology is normal, but there is an intrinsic problem in the colonic motility. Although sacral neurostimulation is currently advised for ISTC due to certain disadvantages, it is not a preferred technique. In the present study; it is aimed to investigate the efficacy of posterior tibial nerve stimulation in the treatment of ISTC. Patients and Methods: Stimulation was performed on 10 patients who were diagnosed to have left colonic ISTC. Constipation level was determined using Wexner scoring system. Patients without any history of anorectal surgery; and in whom conservative measures and biofeedback had failed were included in the study. Patients were evaluated with a through history and physical examination as well as endorectal ultrasonography, anorectal phyiologic tests and barium enema colon graphies. Colonic stimulation prior to stimulation was measured. Posterior tibial nerve stimulation was performed at 0.5-10 mA range with a 200 μs width current at a 20 Hz frequency. At the end of the first year of the stimulation; Wexner scoring, anorectal physiology and colonic transit was recalculated. Results: There was a significant improvement in the anorectal physiology, Wexner scores and colonic transit followingthe posterior tibial nerve stimulation. Conclusion: Posterior tibial nerve stimulation is an indirect sacral nerve stimulation method can be comparable and less invasive than direct sacral nerve stimulation in the treatment of ISTC of the left colon.

Amaç: İdiopatik yavaş geçişli konstipasyon (İYGK) kolon ve rektum morfolojik olarak normal olmasına rağmen görülen bir konstipasyon şeklidir ve primer neden kolonik motilitede oluşan bozukluklardır. Sol kolona ait İYGK tedavisinde direkt sakral nöromodülasyon tekniği birçok çalışmada etkili bir tedavi seçeneği olarak önerilmesine rağmen, bazı dezavantajlara sahip olması nedeniyle tedavide belirgin bir kullanım alanı bulamamıştır. Bu çalışmada posterior tibial sinir stimülasyonunun İYGK tedavisindeki etkinliğinin araştırılması amaçlanmıştır. Hastalar ve Yöntem: Stimülasyon işlemi sol kolona ait İYGK tanısı konulan 10 hastaya uygulandı. Konstipasyon derecesi Wexner skorlaması kullanılarak belirlendi. Herhangi bir anorektal cerrahi işlem geçirmemiş, konservatif yöntemler veya biofeedback tedavisi başarısız olan hastalar çalışmaya alındı. Hastalar prospektif olarak detaylı hikaye ve klinik muayene yanında endorektal ultrasonografi, anorektal fizyoloji testleri ve baryumlu kolon grafileri ile değerlendirildi. Stimülasyon öncesinde kolonun geçiş zamanı ölçüldü. Posterior tibial sinir stimülasyonu 0.5-10 mA aralığında, 200 μs genişliğinde bir akım ile 20 Hz’lik frekansta yapıldı. Stimülasyon sonrası hastalar en az 1 yıl süre ile takip edildi. Birinci yılın sonrasında Wexner skorlaması, anorektal fizyoloji testleri ve kolonun geçiş zamanı işlemleri tekrarlandı. Bulgular: Stimülasyon sonrası yapılan Wexner konstipasyon skorlaması, anorektal fizyoloji testleri ve kolonun geçiş zamanı değerlerinde stimülasyon öncesi yapılan değerlendirmelere göre belirgin düzelmelerin olduğu tespit edildi (p
