The short time effects of calcium dobesilate,diosmin-hesperidin and/or sitz bath on acute complaints in the treatment of patients with bleeding hemorrhoidal attacks

Abstract
Purpose: Rectal bleeding and perianal pain/discomfort are common indications for acute hospital admissions to the general surgery department. The patients consult to the doctor early because of the scary effect of bleeding and decrease quality of life by pain. In our study we compare the effectiveness of calcium dobesilate (CD), diosmin-hesperidin (MPFF) and sitz bath (SB) that were used alone and all together at the acute times of therapy. Method: 173 consecutive patients were enrolled to our study with rectal bleeding. As a prospective randomized study, there were 5 groups participating each of them at least 34 patients: These are MPFF, MPFF+SB, CD, CD+SB and SB only. For discomfort and physical signs, the patients were seen at the hospital again at the 3th and 10thdays.Results: The median age of the patients was 28(16-56) years. Seventy percentages of the patients had Grade II and %30 of them Grade III hemoroids. Significant discomfort/ pain relief was seen almost all in 5 groups (the best one was MPFF+SB group). The minimum effectiveness was seen on CD alone group and SB group. Sitz bath was used by 3 of the groups and rectal bleeding was detected only at 2 (%1.9) patients in these groups. Conclusion: At the patients with painfull Grade II and grade III hemorroidal diseases, oral therapy and sitz bath alone are not enough to give adequate acute response. The best clinical response is got out from the usage of oral therapy and sitz bath all together

Amaç: Rektal kanama ile cerrahi kliniğine başvuran hastaların konservatif yöntemlere erken dönemde vermiş oldukları yanıt araştırılmıştır. Rektal kanama ve perianal ağrı cerrahi polikliniklere en sık başvuru sebeplerindendir. Kanama tedirgin edici olması ve ağrının hayat kalitesine olumsuz etki yapması doktora başvurmada etki sebeplerdendir. Çalışmamızda, evre II ve III hemoroid hastalarında akut dönemde kalsiyum dobesilat (KD), diosmin-hesperidin (MPFF) ve oturma banyosunu (OB) tek başlarına ve birlikte kullanarak akut dönemde tedavideki etkinliklerini değerlendirdik. Yöntem: Cerrahi polikliniğine rektal kanama ile gelen ve fizik muayenelerinde hemoroidal rahatsızlığı tespit edilen, başkaca perianal rahatsızlığı olmayan ve endoskopide patoloji tespit edilmemiş 173 hasta çalışmamıza dahil edilmiştir. İleriye dönük randomize olarak ve herbiri en az 34 olgu içeren 5 grup: MPFF, MPFF+OB, KD, KD+OB ve OB olmak üzere oluşturulmuştur. Hastalar daha sonra 3.günde çağrılarak, şikayet ve bulgulardaki değişim tespit edilip karşılaştırılmıştır, 10.günde tekrar kontrol edilmişlerdir. Bulgular: Olguların ortalama yaşı 28 (16-56). Şikayetlerin ortalama başlangıç zamanı 2.3 gündü. Ağrı %85 olguda kanamaya eşlik ediyordu. Olgulardan %70’inde grade II, %30’unda evre III hemoroid vardı. Beş grupta da ağrı şikayetinde (en çok MPFF+OB grubunda olmak üzere) belirgin azalma oldu, en az fayda görenler sadece KD ve OB kullanan olgulardı. Oturma banyosu alan 3 gruptada da kanama sadece 2 (%1.9) olguda görülürken, sadece drog alanlarda 19 (%26) olguda tedavi sonucu kanama devam etmiştir. Sonuçlar: Evre II ve III ağrının eşlik ettiği kanamalı hemoroid olgularında oral tedavi ve oturma banyosu tek başlarına istenen akut cevabı sağlamada yetersiz kalmıştır. Oral tedavinin oturma banyosu ile desteklenmesi en iyi klinik yanıtı ortaya çıkartmıştır.
