The infections of the neutropenic patients and the value of surveillance cultures of microbial colonization for their diagnosis

Abstract
This study was performed between November 1993 and February 1995, for a period of sixteen months in Osmangazi University Faculty of Medicine, Department of Haematology. We followed 62 inpatients with 126 neutropenic periods. We observed the patients neutropenic periods by collecting their pharyngeal, urine and stool specimens for culturing twice a week. Besides, blood samples were collected in the febril periods and catheter samples once a week for culturing. Fever was present in 57 periods. These patients with febril neutropenic periods were classified by their neutrophyl counts and the first groups with 500-1000 mm3 neutrophyls was shown to be in 3 febril periods, and the second groups with 200-500/mm3 neutrophyls was shown to be in 10 periods, and third groups with less than 200/mm3 neutrophyl count was shown to be in 44 febril periods. Growth was obtained in the blood cultures of 19 febril neutropenic periods. In 19 febril periods, the patients showed septic syndrome and fourteen of them were lost. The septic patients infection site was shown to be the most frequently the lower respiratory tract, followed by enteritis, catheter injection and rectal abscess (one patient). Blood culturing results yielded Cram-positive bacteria (42.1 %) and gram -negative bacteria (36.8 %) and yeasts (10.5 %). In 49 % of the febril neutropenic periods (in 33 % blood cultures) bacterial growth was obtained. Of Gram-positive bacteria, coagulase negative staphylococci: of Gram-negative bacteria Klebsiella pneumoniae and of yeasts Candida spp. were the most common isolates. As a result of surveillance cultures, growth of pathogen microorganisms was obtained especially by culturing blood and sputum samples.

Bu çalışmada, Kasım 1993- Şubat 1995 tarihleri arasında 16 ay süresince Osmangazi Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı'nda nötropeni nedeniyle Yatırılarak takip edilen 62 hastanın 126 nötropenik süreci izlendi. Hastaların nötropenik süreçlerinde haftada iki kez alınan boğaz, idrar ve gaita kültürleri takip edildi. Ayrıca hastaların ateşli dönemlerinde kan ve haftada bir kez de kateter kültürleri alındı. Bu izlemde 57 süreçte ateş saptandı. Ateşli hastaların nötrofil sayılarına göre dağılımı, nötrofil sayısı(PMNL) 500-1000/mm3 olan grupta 3 süreç, PMNL 200-500/mm3 olan grupta 10 süreç, PMNL < 200/mm3 olan grupta 44 süreç olarak belirlendi. Hastaların 19 sürecinde kan kültüründe üreme elde edildi. İzlenen ateşli hastalardan 19 süreçte tablo septik sendroma ilerledi ve bu hastalardan 14'ü kaybedildi. Sepsisli hastalarda infeksiyon odağı olarak en sık alt solunum yolu injeksiyonu, daha nadir olarak enterit, kateter infeksiyonu ve bir olguda rektal abse saptandı. Kan kültürlerinde Gram pozitif bakteriler % 42.1, Gram negatif bakteriler %36.8, mayalar %10.5 oranlarında üredi. Kültür izlemleri sonucu febril nötropenik hastaların bu süreçlerinin % 49'unda üreme (kan kültüründe % 33) elde edildi. En sık izole edilen etkenler Gram pozitif bakterilerden koagulaz negatif stafilokoklar. Gram negatif bakterilerden Klebsiella pneumoniae, mantarlardan da Candida türleri olarak belirlendi. Gelişen infeksiyonlar yönünden incelendiğinde, izlemde en çok kan ve balgam kültürlerinde üreme olduğu gözlendi.
