The effect of nitric oxide on testis ischemia-reperfusion injury

Abstract
An experimental study was conducted on 33 male Wistar rats to investigate the effects of endogenous nitric oxide on the reperfusion injury of testis Testicular ischemia was achieved by twisting the right testis and the spermatic cord 1080$circ$ counter-clockwise for 30 minutes and reperfusion was allowed for 30 minutes after detorsion. The tissue damage was evaluated with light microscopy, malondialdehyde (MDA) level in tissue determined by thiobarbituric acid reaction and the blood flow measurement using xenon clearence technique. NG-nitro-L-arginine methyl esther (L-NAME) was given intravenously just before the detorsion of the testis for the inhibition of NO synthesis at 10mg/kg single dose and L-arginin infusion at 100mg/kg/hour rate was employed during reperfusion in the others to stimulate NO synthesis, both to be compared to controls given only normal saline before the detorsion. Blood flow was determined before the torsion, during the ischemia and after detorsion by xenon clearence technique among three groups each composed of five subjects. Animals in groups were treated with either normal saline or L-NAME or L-arginin just before detorsion. Basal blood flow variations in groups were not significant where as it was 1/10th of basal value during torsion. Blood flow after detorsion could reach 63% of basal in normal saline group, 59% in L-NAME group and it was significantly higher in L-arginine group as 76% of basal. Six groups were assigned for MDA determination and histopathological examination. Animals were treated with normal saline in controls just before detorsion, L-NAME and L-arginine in others. Removed right testes constituted experimental groups and left testes control groups. MDA, indicator of reperfusion injury increased 25% after detorsion when only normal saline was given, L-NAME further increased MDA, L-arginine decreased MDA to control level. Histopathological findings were better in L-arginine group though statistically insignificant. In conclusion L-arginine infusion during the detorsion reduced the reperfusion injury of testis and improved the testicular blood flow after the detorsion.

Tesliste iskemi-reperfüzyon hasarına endojen nitrik oksitin (NO) etkilerini incelemek için otuzüç Wistar cinsi erkek sıçan kullanıldı. İskemi, sağ testiste ve spermatik kordonda 30 dakika süreyle saat yönü tersine 1080 derece torsiyon ile sağlandı. Detorsiyondan sonra 30 dakika reperfüzyon yapılarak xenon klirens tekniği ile testis kan akımı ve tiobarbitürik asit yöntemiyle testis dokusunda malondialdehid (MDA) ölçülüp, ışık mikroskobu ile histopatolojik inceleme yapıldı. Detorsiyondan hemen önce NO sentezini inhibe etmek için intravenöz 10mg/kg tek dozda NG-nitro-L-arginin metil ester (L-NAME); endojen NO sentezini arttırmak içinse reperfüzyon süresince saatte 100mg/kg L-arginin infiizyonu yapılırken kontrollerde 1 mililitre %0.9 NaCl çözeltisi (serum fizyolojik) intravenöz verilmiştir. Kan akımı ölçümü için oluşturulan beşerli üç grup sıçan 'a serum fizyolojik, L-NAME ve L-arginin uygulanarak testis bazal, torsiyon ve detorsiyon sonrası kan akımları ölçüldü. Bazal kan akımlarında anlamlı fark saptanmazken torsiyon sırasında kan akımı bazal değerlerin 1/10'una düşmüştür. Reperfüzyonda ise serum fizyolojik grubunda kan akımının bazal değerin %63'ü, L-NAME grubunda %59'u olduğu ve L-arginin grubunda ise anlamlı artışla bazal değerin %76'sına kadar ulaştığı saptandı. Reperfüzyonda serbest oksijen radikali oluşumu ve reperfüsyon hasarını gösteren lipid peroksidasyonun son ürünü MDA, serum fizyolojik uygulanıp reperfüzyon yapıldığında %25 oranında artmış, L-NAME uygulanması MDA oluşumunu arttırırken L-aginin infiizyonu MDA'yı kontrol düzeyine indirmiştir. Histopatolojik incelemede L-arginin uygulandığında olumlu değişiklikler saptanmış ancak istatistiksel olarak anlamlı bulunmamıştır.
