Pankreasın müsinöz kistik neoplazmı ve kist sıvısında artmış tümör marker düzeyi (Olgu sunumu ve literatürün gözden geçirilmesi)

Abstract
Cystic lesions of the pancreas are quite rare and mucinous cystic neoplasm is 10 % of benign pancreatic tumors, and cystadenocarcinoma is 1% of malignant pancreatic tumors. Indeed, an accurate preoperative and even postoperative classification of these tumors could be difficult. Radiological and clinical studies are not sufficiently reliable. Recently, interest has focused on preoperative and postoperative cyst fluid analysis in the differential diagnosis of cystic lesions of the pancreas. A female patient who is thirty-seven years old admitted to the hospital with stomach-ache, dyspepsia and distention. On examination, a giant cystic mass was palpated in the left upper abdomen. CT and ultrasound showed a cystic mass with 135-mm in diameter, placed in the corpus of pancreas. The various parameters of cystic fluid have been evaluated. Serum tumor marker levels were normal, however the level of tumor markers (CEA, CA 15-3, CA19-9, CA 125) except AFP were found to be very high in the cystic fluid. Total cystectomy with distal pancreatectomy and splenectomy were performed. Histopathologic diagnosis was mucinous cystic neoplasm. Postoperative period was normal, and the patient was discharged eight days after the operation and was followed for one and a half-year without recurrence. There are many investigations explaining the correlation between tumor marker levels of cystic fluid and malignancy potential of pancreatic cyst in the literature. Herein, we described and discussed a mucinous cystadenoma of the pancreas in view of the literature.

Pankreasın kistik lezyonları oldukça nadirdir ve benign pankreatik tümörlerin % 10' unu, kistadenokarsinoma da malign pankreatik tümörlerin % 1 ' ini oluşturur. Gerçekte, bu tümörlerin preoperatif hatta postoperatif klasifikasyonu güç olabilir. Radyolojik ve klinik çalışmalar yeterli değildir. Son günlerde preoperatif ve postoperatif kist sıvısı analizine yönelmiştir. 38 yaşında bir bayan hasta karın ağrısı, dispepsi ve distansiyon ile hastaneye başvurdu. Muayenede sol üst abdomende dev kitle palpe edildi. Computerised Tomography (CT) ve Ultrasound (US ), pankreas korpusuna yerleşmiş 135 mm kistik bir kitlenin varlığını gösterdi. Kistik sıvıda değişik parametreler araştırıldı. Serum tumor marker düzeyleri normaldi, bununla birlikte, AFP dışındaki diğer tümör markerları (CEA, CA 15-3, CA 19-9, CA 125) çok yüksek bulundu. Distalpankreatektomi, splenektomi ile birlikte kist total olarak çıkarıldı. Histopatolojik tanı müsinöz kistik neoplazmdı. Postoperatif dönem normaldi ve hasta postoperatif 8. gün taburcu edildi. Ameliyattan sonra 1.5 yıl nüks olmaksızın takip edildi. Literatürde pankreatik kistlerin malignite potansiyeli ile pankreatik kist sıvısı tümör marker düzeyi arasındaki ilişkiyi gösteren pek çok çalışma vardır. Burada, literatürün ışığında, müsinöz kistadenom vakasını tanımlamak ve tartışmayı amaçladık.
