Isolate spinal cord involvement in neuro-Behçet syndrome: Case report

Abstract

An 18 years old female patient applied to our clinic with the complaints of sudden headache with anesthesia on left arm and leg. The neurological examination findings were right hemihypoesthesis and bilateral hyperactive deep tendon reflexes. She has the findings of uveitis sequel. There was no pathological findings on the cranial, thoracal and lomber MRI, however in the cervical MRI there were hyper intense on T2 weighted images and hypo intense on Tl weighted images ring shaped lesions at the level of fourth and sixth cervical vertebras, which were consistent with the Behçet's lesions. The lesions were absent on the control MRI taken one month later while immunosuppressive treatment is maintained. We decided to report this neuro-Behçet syndrome with an isolated medulla spinalis lesion without a cranial MRI lesion because it's a rare entity.

Özet

On sekiz yaşında kadın hasta ani gelişen şiddetli baş ağrısı, sol kol ve bacağında his kaybı şikayetleri ile başvurdu. Nörolojik muayenesinde sağ hemihipoestezi mevcut olup, bilateral derin tendon refleksleri canlı idi. Göz muayenesinde geçirilmiş üveit bulguları vardı. Kranyal, torakal ve lomber manyetik rezonans görüntüleme (MRG) tetkiklerinde patolojik bulgu görülmezken; servikal MRG'de medulla spinaliste dördüncü ve altıncı servikal vertebralar seviyesinde, ring tarzında, T2W'de hiperintens, Tl W'de hipointens iki adet Behçet lezyonu ile uyumlu patolojik odak görüldü. İmmunosupresif tedavi altında iken bir ay sonra çekilen kontrol servikal MRG'de bu lezyonlann kaybolduğu görüldü. Behçet hastalığı (BH) tanısı almış, kranyal MRG bulgusu olmayan, izole medulla spinalis tutulumu gösterilen nöro-Behçet Sendromu olgusunu nadir görülmesi nedeniyle sunmayı uygun bulduk.
