Lipid profile of diabetic patients: awareness and the rate of treatment success

Abstract
Background: Trials including diabetic and pre-diabetic pa- tients have shown that the risk of cardiovascular event is in- creased in patients in whom blood glucose levels are high. In this study, we analyzed the achieval rate of target cholesterol levels in patients with diabetes mellitus. Methods: 344 diabetic patients admitted to our hospital be- tween March and May 2012 were randomly included. Low density lipoprotein (LDL), high density lipoprotein (HDL) and triglycerides (TG) were measured in our hospital biochemistry laboratory. Patients’ treatments were recorded. Results: Although all 135 male and 209 female diabetic pa- tients were undergoing treatment for diabetes, 83% (n=286) had LDL cholesterol levels greater than 100mg/dl. Despite this, only 47.3% (n=163) were taking lipid-lowering medica- tions. The mean level of LDL-cholesterol was: 126.4±33.5 mg/ dl, TG: 179.2±6.53 mg/dl; HDL-cholesterol: 45.9±10.8 mg/dl.In 16.8% of the patients (n=58) target LDL cholesterol was achieved. In addition, all lipid targets (LDL 50 mg/dl in women) were achieved in nearly 5% (n=17) of the patients. Conclusion: This study, which was done in a randomly select- ed group of follow-up diabetic patients, showed that we are far away from lipid targets. We must approach more aggressive for treatment of hyperlipidemia.

Amaç: Diyabetik ve prediyabetik hastalarda yapılan çalış-malar kan glukoz seviyeleri yüksek seyredenlerde kardi- yovasküler olay riskinin artmış olduğunu göstermiştir. Buçalışmada diyabet tanılı hastalarda hedeflenen kolesterolseviyelerine ulaşma oranımızı inceledik.Gereç ve Yöntem: 2012 yılı Mart-Mayıs ayları arasında has- tanemize başvuran 344 diyabetik hasta randomize olarakçalışmaya alındı. Düşük yoğunluklu kolesterol (LDL), yük- sek yoğunluklu kolesterol (HDL) ve trigliserit (TG) değerlerihastanemiz biyokimya laboratuvarında ölçüldü. Hastaların aldıkları tedaviler kaydedildi.Bulgular: Diyabet tanılı ve tedavi almakta olan 135 erkek, 209 kadın hastanın %83’ünün (n=286) LDL kolesterol seviyesi 100mg/dl üzerinde olmasına rağmen, sadece %47.3’ü (n=163) lipit düşürücü tedavi almakta idi. Ortalama LDL-kolesterol düzeyleri: 126.4±33.5 mg/dl, TG düzeyleri: 179.2±6.53 mg/ dl, HDL-kolesterol düzeyleri: 45.9±10.8 mg/dl idi. Hastaların %16.8’inde (n=58) LDL kolesterol hedefine ulaşılabilirken, bu- nun yanı sıra tüm lipit hedeflerine (LDL 50 mg/dl [kadın]) ulaşılabi-len hastalar ise tüm hastaların yaklaşık %5’i (n=17) idi.Sonuç: Takibimizde olan diyabetik hastalardan randomizeseçilen bir grupta yaptığımız bu çalışma göstermiştir ki lipit hedeflerinden uzaktayız. Diyabeti olan hastalarda hiperlipi-demi tedavisine daha agresif yaklaşmalıyız.
