Seroprevalence of hepatitis b and c among women with fibromyalgia syndrome

Abstract
Background:Fibromyalgia syndrome (FMS) is one of the major health problems with an unknown etiology. Trigger factors in cluding various infectious agents may play a role in the develop ment of the disease. The aim of this study was to investigate the seroprevalence of hepatitis B and C among patients with FMS. Methods:Fifty-seven female patients diagnosed with FMS ac cording to the American College of Rheumatology (ACR) 1990 criteria were included in the study. Demographic features of the patients as well as duration of widespread pain, severity of pain as sessed with visual analog scale (VAS), and total number of tender points (NTP) were recorded. Fibromyalgia Impact Questionnaire (FIQ) was also performed. Sera of the patients were examined with micro-ELISA method for the presence of HBsAg, anti-HBs and anti- HCV. Regarding exposure to HBV and/or HCV, the patients were separated into two groups as exposed and non-exposed. Results:Of the 57 patients, four were HBsAg-positive (7%) and 13 were anti-HBs- positive (22.8%). None of the patients was positive for anti-HCV. Scores of NTP, VAS and FIQ were higher in the HBV- exposed group than in the non-exposed group, but there were no statistically significant differences between the two groups. Conclusion:No relationship was determined between FMS and HBV and HCV infections in this study. However, the severity of FMS symptoms was higher in the HBV-exposed group than in the non-exposed group. Further multidisciplinary studies in cluding larger samples may help to determine the seropreva lence of HBV and HCV in FMS.

Amaç:Fibromiyalji sendromu (FMS) etiyolojisi tam olarak anlaşılamamış önemli sağlık problemlerinden biridir. Has talığın oluşmasında enfeksiyöz ajanları da içeren tetikleyici faktörler rol oynayabilmektedir. Bu çalışmada FMS tanısı alan hastalarda hepatit B virüsü (HBV) ve hepatit C virüsü (HCV) seroprevalansının araştırılması amaçlanmıştır. Gereç ve Yöntem:Amerikan Romatoloji Derneği (ACR) 1990 ölçütlerine göre FMS tanısı almış 57 kadın hasta çalışmaya alındı. Demografik bilgilerin yanı sıra yaygın vücut ağrısının süresi, görsel ağrı skalası (GAS) ile ağrının şiddeti ve toplam hassas nokta (HN) sayısı kaydedildi. Hastalara fibromiyalji etki anketi (FEA) uygulandı. Hastalarda mikro-ELİSA yöntemi ile HBsAg, antiHBs ve antiHCV varlığı araştırıldı. HBV ve/veya HCV maaruziyetine göre hastalar karşılaşanlar ve karşılaşma yanlar olarak iki gruba ayrıldı. Bulgular:Hastalarda HBsAg pozitifliği %7 (4/57), anti HBs pozitifliği ise %22.8 (13/57) oranında saptandı, anti-HCV pozitifliğine rastlanmadı. HBV ile karşılaşan hastalarda HN sayısı, GAS ve FEA skorları daha yüksekti, ancak aradaki fark istatistiksel olarak anlamlı değildi. Sonuç:Bu çalışmada FMS ile HBV ve HCV enfeksiyonu ara sında net bir ilişki saptanmadı. Ancak FMS semptomlarının şiddeti HBV ile karşılaşanlarda daha belirgindi. Çok merkezli, büyük örneklem sayıları ile yapılacak ileri çalışmalar FMS’de hepatit B ve C seroprevalansının ve hastalık şiddeti ile ilişki sinin belirlenmesinde daha faydalı olacaktır.
