Comparison of the efficiency of fifty gram and seventy gram oral glucose tolerance tests in the diagnosis of gestational diabetes mellitus

Abstract
Objcctives:To compare the efficiency of fifty and seventyfive gram oral glucose tolerance testsin the diagnosis of gestational diabetes mellitus (GDM) and in the prediction of perinatal outcomes. Materials and Methods: One hundered eightyfive pregnant women of 24-28 weeks gestation whom admitted for routine prenatal visits, between August-October 2002 were included in the study. A detailed history including the risk factors for GDM was taken and patients were randomized into two groups. Patients recorded with odd numbers (group 1, n=98) were given 50 gr oral glucose and if first hour venous plasma glucose value exceeded 140mg/dl, 100 gr oral glucose tolerance test (OGTT) was performed . In group 1, if any two measured values exceeded cut-off levels , those patients were diagnosed as GDM according to NDDG (National Diabetes Data Group) criteria. Patients recorded with even numbers (group 2, n=74) were given 75 gr OG load, and categorized as impaired glucose tolerance (IGT), if the second hour value exceeded 140 mg/dl according to WHO criteria. Thirteen cases that have delivered outside the institution were excluded from the study. In both groups, perinatal and neonatal outcomes of cases diagnosed to have GDM or IGT, were compared with normal subjects. Results: Mean age, body mass index (BMI), gravity, parity, prevelance of preterm labor and cesarean delivery, gestational age at birth and apgar scores were not statistically different between the two groups. The rate of family history of diabetes were statistically different between the two groups (p= 0.035). Six patients in group 1 (% 6) were diagnosed to have GDM and six patiens in group 2 ( % 8) were diagnosed as IGT. In group I cases with GDM were significantly higher mean BMI than controls (p=0.002). Besides in group I, cases with GDM have higher rates of polyhydramnios than controls (p=0.008). These macrosomia and hypoglycemia risks were found to be higher in GDM and IGT cases than normal controls. In both groups, there was neither intrauterine nor neonatal fetal deaths. Conclusions: WHO criteria were found to be equally predictive for adverse perinatal outcomes as well asNDDG criteria. Seventyfive gram OGTT is a one step test, easy to apply and has the advantage of direct comparison with standart OGTT in non pregnant state.

Amaç: Gestasyonel diabetes mellitus (GDM) tanısı ve perinatal sonuçların prediktivitesi açısından 50gr ve 75gr oral glukoz tolerans testlerinin etkinliğinin karşılaştırılması. Materyal ve Metod: Bu çalışma Ağustos-Kasım 2002 tarihleri arasında hastanemiz antenatal polikliniğine başvuran, 24-28 gebelik haftaları arasında olan 185 sağlıklı gebe üzerinde yapıldı.GDM açısından risk faktörleri sorgulanan olgular randomize olarak iki gruba ayrıldı. Grup l'e (n=98) 50gr glukoz yükleme testi ve venöz plazma glukoz değeri 140 mg/dl üzerinde olanlara lOOgr oral glukoz tolerans testi (OGTT), Grup 2'ye (n=74) 75gr OGTT uygulandı. Grup l'de lOOgr OGTT sonueunda açlık 105 mg/dl, 1 saat 190 mg/dl, 2.saat 165 mg/dl, 3.saat 145 mg/dl kestirim değerlerinden en az iki değeri yüksek bulunan olgulara National Diabetes Data Group (NDDG) kriterlerine göre GDM tanısı konuldu. Grup 2'de 2. saat plazma glukoz değeri 140mg/dl üzerinde olanlara WHO kriterlerine göre anormal glukoz toleransı(AGT) tanısı konuldu. Hastanemizde doğurmayan 13 olgu çalışma dışında tutuldu. GDM ve AGT tanısı alan ve almayan olgular perinatal ve neonatal özellikler açısından karşılaştırıldı. Sonuçların karşılaştırılmasında ki-kare,bağımsız örneklerde t-testi ve rölatif riskler kullanıldı. Bulgular: İki grup arasında yaş, BMI(vücut kitle indexi), gravida, parite, preterm eylem, sezeryan oranı, doğum haftası, apgar skorları açısından anlamlı bir fark bulunmadı. Ailede diabet öyküsü açısından her iki grup arasında istatistiksel olarak anlamlı bir farklılık saptandı (p=0.035). Grup l'de 6 olgu (% 6) GDM tanısı alırken Grup 2'de 6 olgu AGT tanısı aldı (% 8). Grup l'de GDM tanısı alanların BMI'leri normal olanlara göre istatistiksel olarak anlamlı oranda yüksek bulundu (p=0.002). Yine Grup 1 'de GDM tanısı alan olgularda polihidramnios oranı istatistiksel olarak anlamlı oranda yüksek bulundu (p=0.008). Her iki grupta da makrozomi ve hipoglisemi riski normal olgulardan yüksek bulundu. Her iki grupta da intrauterin fetal exitus saptanmadı. Sonuçlar: WHO kriterleri anormal perinatal sonuçların (özellikle makrozomi) prediktivitesinde NDGG kriterleri kadar etkin olduğu saptanmıştır Bu çalışma sonucunda, uygulama kolaylığı, tanı için 100 gr OGTT gibi ikinci bir testi gerektirmemesi ve gebelik sonrası takiplerde uygulanılacak standart bir test olması nedeniyle 75 grOGTT'nin kullanılması önerilmektedir.
