Second trimester maternal serum HCG, AFP, estriol concentrations in ICSI and IUI pregnancies for down's syndrome screening

Abstract

Background: The aim of this study was to evaluate second trimester maternal serum total human chorionic gonadotrophin (hCG), alpha- feto protein (AFP) and unconjugated estriol ( $uE_3$ ) concentration in singleton pregnancies conceived from intracytoplasmic sperm injection (ICSI) and intrauterine insemination (IUI). Also, the influence of the number of embryos transferred and the amount of gonadotrophins used on the marker levels was evaluated in the ICSI pregnancies. Methods: Second trimester (16-20 weeks) maternal serum levels of AFP, $uE_3$ , and hCG were measured and expressed as multiples of median (MoM) values in 32 ICSI and 28 IUI conceived singleton pregnancies. The results were compared with those of 156 naturally occuring singleton pregnancies who had serum screening at 16-20 weeks at the same laboratory over the same period of time. .Results: There were no significant differences between the assisted reproductive technology (ART) pregnancies and control group in maternal age, body mass index (BMI), gestational week and habitus of cigarette smoking. In assiste reproductive technology (ART) patients, parity was lower than that of the controls. No significant differences of maternal serum markers could be observed between pregnancies following IUI and controls . In ICSI pregnancies, the values of AFP and $uE_3$ were not significantly different from that in controls, but total hCG was significantly increased. No women were found to be screen positive. In neither group were chromosomal abnormalities detected and no fetal or neonatal deaths were recorded. There were no correlation between marker levels with the number of transferred embryos and amount of gonadotrophins used in ICSI pregnancies. Conclusions: Based on our findings, IUI did not significantly affect the results of maternal serum marker levels. Maternal serum hCG levels during the second trimester is elevated in pregnancies following ICSI. The increased hCG levels may contribute to the higher Down's Syndrome screen positive rate in this ICSI singleton population. Further studies are needed to determine the best screening method for Down's Syndrome in assisted pregnancies.

Amaç: Bu çalışmanın amacı, intrasitoplazmik sperm enjeksiyonu (ICSI) ve intrauterin inseminasyon (IUI) kullanılarak oluşturulmuş tekil gebekliklerin ikinci trimesterinde maternal serum total human chorionic gonadotrophin (hCG), alpha-fetoprotein (AFP) ve unconjugated estriol (E3) konsantrasyonlarını değerlendirmektir. Bunun yanında ICSI gebeliklerinde transfer edilen embyro sayısının ve kullanılan gonadotrophin miktarının marker düzeylerine etkileri de değerlendirilmiştir. Materyal ve metod: İkinci trimester (16-20 haftalar) maternal serum AFP düzeyleri, E3 ve hCG düzeyleri ölçüldü ve 32 ICSI ve 28 IUI tekil gebelikte çoklu median değerler (multiples of median values) belirtildi. Bu sonuçlar 16.-20. haftalarda aynı laboratuarda serum değerleri ölçülen 156 doğal tekil gebeklikle karşılaştırıldı. Bulgular: Assisted reproductive technology (ART) gebelikleri ile kontrol grubu arasında maternal yaş, vücut kitle indeksi, gestasyonal hafta ve sigara içme alışkanlığı açısından anlamlı farklılık yoktu. ART hastalarında parite kontrol grubundan daha düşüktü. ICSI gebeliklerinde AFP ve E3 değerleri kontrol grubu değerlerinden anlamlı düzeyde farklı değildi fakat total hCG düzeyi anlamlı olarak artmıştı. Hiçbir kadının üçlü testi screen pozitif olarak bulunmadı. Her iki grupta da kromozomal anomali tespit edilmedi ve fetal ya da neonatal ölüm kaydedilmedi. ICSI gebeliklerinde marker düzeyleri ile transfer edilen embriyo sayısı ve gonadotrophin miktarları arasında herhangi bir korelasyon yoktu. Tartışma: Bulgularımıza göre IUI maternal serum marker düzeylerini anlamlı olarak etkilememektedir. ICSI yi takip eden gebeliklerde ise maternal serum hCG düzeyleri ikinci trimester süresince yükselir. Yükselmiş hCG düzeyleri ICSI populasyonunda daha yüksek Down Sendromu screen pozitif oranlarına katkıda bulunuyor olabilir.Assisted gebeliklerde Down Sendromu için en iyi tarama metodunu tespit etmek için daha fazla çalışmaya ihtiyaç vardır.
