Role of transvaginal ultrasonography, hysterosonography and hysteroscopy in the diagnosis of abnormal uterine bleeding

Abstract

Objective: To evaluate the diagnostic accuracy of transvaginal ultrasonography, sonohysterography and diagnostic hysteros-copy in premenopausal and postmenopausal patients with abnormal uterine bleeding. Materials and Methods: In this prospective randomised study, women who complained of abnormal uterine bleeding, 84 ca¬ses were evaluated from October 1 2002 to December 31 2003. The patients' ages ranged 25 to 77 years. The patients were divided into two groups. Group A (42 patients) underwent transvaginal ultrasonography, group B (42 patients) underwent sonohysterography. All patients then had office diagnostic hysteroscopy or operative hysteroscopy. The final diagnosis was made by operative hysteroscopy with resection and excision of the lesions or endometrial biopsy with vacum curettage. Sen¬sitivity, specificity, positive predictive value and negative predictive value of transvaginal ultrasonography, sonohyste¬rography and diagnostic hysteroscopy were calculated. Results: The mean age was 49.1 years. Fourty nine patients were in premenopausal, and 35 were in postmenopausal period. They complained of abnormal uterine bleeding such as menorrhagia (18 patients), metrorrhagia (12 patients), menometrorr-hagia (17 patients), postmenopausal bleeding (35 patients). The most common causes for the abnormal uterine bleeding we¬re endometrial hyperplasia in transvaginal ultrasonography (61.9%), endometrial polyp in sonohisterography (76.1%), and also endometrial polyp in diagnostic hysteroscopy (52.3%). Transvaginal ultrasonography revealed a sensitivity of 68.1% and specificity of 35%. Sonohysterography revealed a sensitivity of 88.8% and specificity of 50%. Diagnostic hysteroscopy revealed a sensitivity of 83.3% and specificity of 75%. Conclusion: Sonohysterography offer a minimal invazive and cost-effective alternative to diagnostic hysteroscopy in the eva¬luation of patients with abnormal uterine bleeding and can be the most effective method to reduce the number of diagnostic hysteroscopies.

Amaç: Bu çalışmanın amacı, premenopozal ve postmenopozal anormal uterin kanamalı hastalarda, transvajinal ultrasonog¬rafi, histerosonografi ve histeroskopinin tanıdaki rolünü araştırmaktır. Materyal ve Metot: Bu prospektif randomize çalışmada, 1 Ekim 2002-31 Aralık 2003 döneminde anormal uterin kanama şi¬kâyeti ile başvuran, 25-77 yaşlarındaki 84 kadın değerlendirildi. Hastalar, Grup A (42 hasta) ve Grup B (42 hasta) olmak üzere ikiye ayrıldı. Grup A'ya transvajinal ultrasonografik inceleme, Grup B'ye ise sonohisterografik inceleme yapıldı. Daha sonra tüm hastalara ofis diagnostik histeroskopi ya da operatif histeroskopi uygulandı. Histeroskopi sırasında rezeke edilen lezyonların veya küretaj sonrası alınan dokuların patolojik inceleme sonucu konulan tanı, son tanı olarak kabul edildi. Trans¬vajinal ultrasonografinin, sonohisterografinin ve diagnostik histeroskopinin sensitivitesi, spesifisitesi, pozitif prediktif değeri ve negatif prediktif değerleri hesaplandı. Sonuçlar: Yaş ortalaması 49.1 olan 84 hastanın, 49'u premenopozal dönemde, 35'i ise postmenopozal dönemde idi. On sekiz hastada menoraji, 12 hastada metroraji, 17 hastada menometroraji ve 35 hastada postmenopozal kanama şikâyeti vardı. Transvajinal ultrasonografide en sık karşılaşılan bulgu, endometrial hiperplazi (%61.9), sonohisterografide endometrial po¬lip (%76.1), diagnostik histeroskopide de endometrial polip (%52.3) oldu. Transvajinal ultrasonografinin sensitivitesi %68.1, spesifisitesi %35, sonohisterografinin sensitivitesi %88.8, spesifisitesi %50, diagnostik histeroskopinin sensitivitesi %83.3, spesifisitesi %75 olarak bulundu. Tartışma: Sonohisterografi, anormal uterin kanamalı hastalarda, diagnostik histeroskopiye göre uygulaması kolay, zaman al¬mayan ve maliyet açısından daha etkin bir alternatif olabilir ve gereksiz histeroskopik girişimleri azaltabilir.
