Adnexal masses during the pregnancy

Abstract

Summary OBJECTIVE: It is not uncommon to diagnose of an adnexal mass during the pregnancy in an era of prenatal ultrasound. In this study, we reported maternal and perinatal outcomes of persistent adnexal mass associated with pregnancy among women who admitted for antenatal follow-up between 2004 and 2006. MATERIALS-METHODS: Demographic, clinical data and perinatal outcomes of 10 cases of persistent adnexal mass who were diagnosed at 1st and 2nd trimester were retrospectively evaluated RESULTS: Seven of ten were explored surgically. Ultrasonographic aspiration was performed to one case. We followed two cases due to lack of informed consent for the surgical intervention. Final histopathologic diagnosis were available for 8 cases ( 2 cases of dermoid cyst, 3 cases of simple cyst, 1 case of serous cystadenofibroma, 1 case of extragastrointestinal stromal tumor, 1 case of myoma uteri). Pregnancy outcome was uneventful except 3 cases whose pregnancy were terminated. CONCLUSION: Surgical exploration is a suitable option to evaluate persistent symptomatic adnexal masses during the pregnancy. In case of, asymptomatic persistant adnexal masses, sonographic features could be major determinant to decision making.

Özet

AMAÇ: Prenatal ultrasonografinin yaygın olarak kullanımını takiben, adneksiyel kitleler gebede daha sık saptanır hale gelmiştir. Bu çalışmada 2004-2006 yılları arasında, antenatal takip sırasında saptanan ve sebat eden adneksiyel kitlelerin maternal ve perinatal sonuçları tartışılmıştır. Materyal ve Metot: İlk ve ikinci trimesterda tanı alan ve sebat eden 10 olgunun demografik, klinik verileri ve perinatal sonuçları retrospektif olarak gözden geçirilmiştir. BULGULAR: Bu 10 olgudan 7’ si cerrahi olarak eksplore edildi. Bir olguda ultrasonografi eşliğinde aspirasyon uygulandı, 2 olguda ise operasyon için hastadan gerekli onamlar alınamaması nedeni ile cerrahi girişim yapılamadı. Olguların 8’ inde histopatolojik tanıya gidilebildi (2 olgu dermoid kist, 3 olgu basit kist, 1 olgu seröz kistadenofibrom, 1 olgu ekstragastrointestinal stromal tümör, 1 olgu myoma uteri). Gebeliği sonlandırılan 3 olgu hariç, diğer olgularda gebelik seyri sırasında komplikasyon izlenmedi YORUM: Gebelik sırasında persiste eden ve semptomatik olan adneksiyel kitlelerde cerrahi eksplorasyon uygun bir seçenektir. Asemptomatik, persiste adneksiyel kitlelerde ise sonografik özellikler karar vermede başlıca belirleyiciler olabilir.
