Abstract

Delayed diagnosis in a spinal epidural abscess is related to poor outcome. This case highlights an atypical presentation of a spinal epidural abscess. Early diagnosis leads to early therapy and better outcome. We present the case of a man who had a good clinical history and CSF findings to support a diagnosis of subarachnoid haemorrhage.Following a negative cerebral angiogram, a revised diagnosis of ‘spinal’ subarachnoid haemorrhage was made. Further imaging however revealed an epidural abscess.We use this to discuss the investigation of suspected ‘spinal’ subarachnoid haemorrhage. We also discuss the presentation of an epidural abscess and highlight that the clinical presentation may be atypical.

Özet

Spinal epidural absede geç tanı kötü prognozla sonuçlanır. Bu olgu tipik olmayan bir spinal epidural abseyi öne çıkarmaktadır. Erken tanı erken tedavi ve dolayısı ile iyi sonuç demektir. Klinik öykü ve BOS bulguları ile bir subaraknoid kanama tanısını destekleyen erkek bir hasta sunmaktayız. Serebral anjiografi sonucunun negatif olması spinal bir subaraknoid kanama tanısına değişiklik yapmamıza neden oldu. İleri tetkikler bu sefer bir epidural absenin varlığını ortaya koydu. Bu sunumu kuşkulu spinal subaraknoid kanamanın araştırılmasında tartışma konusu yapmak istedik. Aynı zamanda bir epidural absenin atipik bir klinik tablo ile gelebileceğini tartışmada vurgulamak istedik.
