Cavernous Angiomas of The Third Ventricle: A Series of Eight Patients and Review of The Literature

Abstract

Background: Cavernous angiomas are vascular malformations rarely found in the ventricularsystem. Third ventricular cavernous angiomas (TVCA) are rarer and have only beensporadically reported.Methods: We present a series of eight patients with TVCA who were treated in our institute.The clinical data were analyzed retrospectively. Follow-up was obtained based on clinicalobservation. We also analyzed the characteristics and clinical courses of thirty-four cases ofTVCAs that have been reported in the literature.Results: The series includes three males and five females (range: 21 to 38 years; mean 28years). The initial presentation consisted of mass effect in seven cases and acuteintraventricular hemorrhage in one case. None of the patients reported history of seizures. Allpatients underwent direct surgical excision. One patient underwent translamina terminalisapproach, two patients underwent transcallosal transventricular approach and five patientsunderwent transcortical transventricular approach. Total resection was achieved in six patientswhile two underwent subtotal resection. Postoperatively, two patients experienced nocomplications; two patients had electrolyte disturbances; four patients had short-term memorydeficits. All patients received follow-up (mean 49.4 months). The two patients who underwentsubtotal resection had recurrence; one patient who achieved total resection sufferedhydrocephalus postoperatively at six months and had ventriculoperitoneal shunt placed; theremaining five patients had no recurrences.Conclusions: TVCAs are benign lesions; however, symptoms caused by mass effect are oftenthe initial presentation. Microsurgical total resection is the treatment of choice; however, ahigh rate of postoperative short-term memory deficits is still associated with this procedure.

Giriş: Kavernomlar nadiren ventriküler sistemde bulunan vasküler malformasyonlardır.Üçüncü ventrikül kavernomları (3VK) daha da enderdir ve sporadik olarak bildirilmiştir.Yöntem: Kurumumuzda tedavi edilen sekiz 3VK hastası sunulmaktadır. Klinik verilerretrospektif olarak değerlendirildi. İzleme klinik gözleme dayalı olarak yapıldı. Literatürdebildirilen 34 üçüncü ventrikül kavernom olgusu da tarafımızdan klinik seyir ve özellikleriyleincelemeye alındı.Sonuçlar: Bu seri üç erkek ve beş kadından oluşmaktadır (yaşları 21 ila 38 arasında veortalama 28 yaş). Klinik prezentasyon yedi olguda kitle etkisi ve bir olguda akutintraventriküler kanamadır.Hiçbir hastada nöbet öyküsü yoktur. Tüm hastalar direkt olarakcerrahi tedaviye alındılar. Bir hastada translamina terminalis yaklaşımı; iki hastadatranskallozal yaklaşım ve beş hastada transkortikal transventriküler yaklaşım uygulandı. Altıhastada total eksizyon yapılması olası olurken iki hastada subtotal eksizyon yapılabildi.Postoperatif dönemde iki hastada hiç bir komplikasyon olmazken; iki hastada elektrolitdengesizliği; dört hastada kısa-dönem bellek kusurları gelişti. Tüm hastalar ortalama 49,4aylık bir izleme süreci yaşadılar. Subtotal eksizyon uygulanan iki hastada yineleme oldu; totalrezeksiyon uygulanan bir hastada 6 ay sonra hidrosefali gelişti ve ventriküloperitoneal şantuygulandı; geride kalan beş hastada rekürrens gözlenmedi.Yargı: 3VK selim huylu lezyonlardır; kitle etkisi ile oluşturdukları semptomlar sıklıkla ilkbelirtilerdir. Mikrocerrahi ile total eksizyonları tedavi şanslarıdır; ancak operasyon sonrasıyüksek oranda gözlemlenen kısa-dönem bellek bozuklukları bu girişim sonucudur.
