Comparison of Entorhinal Cortex and Hippocampal Volume Measurements of Patients With Mesial Temporal Sclerosis

Abstract
The aim of this study is to evaluate the frequency of EC ( Entorhinal cortex) atrophy inpatients with TLE (Temporal Lobe Epilepsy) demostrate the diagnostic value in patients withor without hippocampal sclerosis.Metod: 23 patients, with TLE who were observed and recording in the Video-EEGmonitorization and a control group of 17 patients were involved in the study. In both goupsdifferences between video recordings of seizures with syncronized EEG recording wereevaluated and lateralization of the seizure focus or the relation with the dominant hemispherewere recorded. 3D-T1 secands hippocampus and EC layers were calculated and mean volümewere calculated.Results: The lateralization signs; versiv head deviation, concurrency of ipsilateral automatismwith contralateral dystonic posture and the 4 sign were found to lateralize the contralateral ofthe seizure focus, while speech arrest lateralize the dominant hemisphere. There were nodifferences in the frequency of lateralization sign between the group of patients withhippocampal sclerosis and the other group without hippocampal screlosis. In both of thepatient groups, frequency of EC atrophy was statistically significant. Although reduction ofEC volümes were frequently bilateral in both groups, EC volümes were found to be smaller atthe ipsilateral seizure focus.Conclusion: The present study demostrated that most of the patients with TLE had ECatrophy. EC volüme measurements with Video-EEG monitoring and neuropsychometric testscan be useful in some circumstanses where it's difficult to distinguish between TLE and ExtraTemporal Lobe Epilepsy and especially to distinguish frontal and temporal seizures.

Amaç: Çalışmamızda EK ( Entorinal korteks) atrofisinin TLE'li (Temporal lob epilepsi)hastalardaki tanı değerini göstermeyi amaçladık.Metod: Toplam 23 temporal lob epilepsili olgu ile 17 kişilik kontrol olgusu çalışmaya katıldı.TLE'li hastalar ilk MR (Magnetik Rezonans) görüntülemelerinde hipokampal atrofi izlenenhastalar ve hipokampal atrofi izlenmeyen hastalar olarak sınıflandırılıp, her iki hastagrubundaki demografik farklar, epilepsi ile ilişkili olabilecek risk faktörleri, sözel ve görselbellek testlerindeki bozukluklar ve her bir nöbet için video görüntülerinden elde edinilengözlemler ile EEG kayıtları ile eş zamanlı değerlendirilerek, nöbet odağına görelateralizasyonu veya dominant hemisfer ile ilişkisi gruplara göre kaydedildi. Bütün hastalardahipokampus ve EK düzeylerinden 3D-T1 volüm sekansları sağ ve sol olarak iki taraflıçalışılıp volüm değerleri ortalamaları ölçüldü.Bulgular: Lateralizasyon bulgularından versiv baş deviasyonu, ipsilateral otomatizma ilekontralateral distonik postür birlikteliği ve 4 işareti' nin hastaların önemli bir kısmında nöbetodağının kontralateralini, konuşmanın durmasının ise dominant hemisferi lateralize ettiğisaptandı. Hipokampal atrofi izlenen ve izlenmeyen her iki grup hastalarda EK atrofi sıklığıistatistiksel olarak anlamlı bulundu. Hipokampal atrofi izlenen hasta grubundaki EK volümdeğerlerindeki küçülme sıklığı hipokampal atrofi izlenmeyen hasta grubundan daha yüksekti.Hasta gruplarında sözel ve görsel bellek testlerindeki bozulmaların nöbet odağı ile ilişkisiistatistiksel olarak anlamlı bulunmadı.Sonuç: Çalışmamızda TLE' li hastaların önemli bir kısmında EK atrofisinin bulunduğugösterildi. Ancak hipokampal atrofinin izlenmediği hastalarda ve TLE ile ETLE ( Ekstratemporal lob epilepsi) hastaların ayırtedilemediği problemli olgularda, özellikle frontal vetemporal nöbetlerin ayırt edilemediği durumlarda EK volüm ölçüm değerlerinin video-EEGve nöropsikometrik testlerle birlikte değerlendirilmesinin nöbet odağının lateralizasyonunayardımcı olabileceği düşünüldü.
