Heat shock protein 27 in migraine patients

Abstract
Aim: The pathophysiological mechanism of migraine is still obscure. Heat shock proteins (Hsp) are stres induced proteins in order to protect cells from stress and apoptosis. As migraine attack may constitute a stressful condition we aimed to look for whether an alteration of Hsp levels of migraine patients during the attacks occur. Material and methods: Twenty-five migraine patients between 20-60 yeras of age were selected randomly from our outpatient clinic. Twenty five healthy subjects with similar ages consisted the control group. Any of the subjects with malignancy, autoimmune disease, infection or fever were excluded from the study. Blood samples were collected during migraine attack and remisssion (at least 7 days of attack free period) in the patient group. Serum Hsp27 levels were examined with Calbiochem® Oncogene kit by using ELISA method. Results: The mean Hsp27 levels were during the attacks 2,63pg/ml, 2,48pg/ml during the remission period and 2,66 ±5,33pg/ml in the control group. Hsp27 levels during attack period was high, but the difference was not statistically meaningful (p=0,80). However the increase of Hsp27 level increase were meaningfully correlated with the Headache Severity Scores (HSS) (p= 0,02). Discussion: In our study, the positive relationship of HSS during headache points that each migraine attack constitute a stress. The reason of nonmeaningful difference of the levels during attack and remission may be related to the smallness of our group.

Amaç: Migrenin patofizyolojik mekanizması hala bilinmemektedir. Isı şık proteinleri (Hsp) hücreleri apoptoz ve stresten korumak üzere ortaya çıkan stres proteinleridir. Migren atağı bir stres durumu yarattığı için migren atağı sırasında Hsp düzeylerinde değişiklik olup olmadığını öğrenmeyi amaçladık. Gereç ve yöntem: Polikliniğimizden yaşları 20-60 arasında 25 migren hastası rasgele olarak seçildi.Benzer yaş grubunda 25 sağlıklı gönüllü kontrol grubunu oluşturdu. Malignite, otoimmun hastalık, enfeksiyon veya ateşi olanlar çalışmadan çıkarıldı. Kan örnekleri hasta grubunda, migren atağı sırasında ve remisyonda ( en az 7 gün ataksız dönemde) toplandı. Serum Hsp27 düzeyleri Calbiochem® Oncogene kiti kullanılarak ELİSA yöntemi ile çalışıldı. Sonuçlar: Hsp27 düzeyleri atak sırasında 2,63pg/ml, remisyonda 2,48pg/ml ve kontrol grubunda 2,66 ±5,33pg/ml idi. Atak sırasında ölçülen Hsp27 düzeyleri yüksekti, ancak fark istatistiksel anlamlılığa ulaşmadı (p=0,80). Bununla birlikte Hsp27 düzeyleri artışı Baş ağrısı Ciddiyet Skorları (BCS) ile anlamlı oranda ilişkiliydi (p= 0,02). Tartışma: Çalışmamızda baş ağrısı sırasında hastalarda, BCS ile anlamlı pozitif ilişki olması her migren atağının zayıf da olsa bir stres oluşturduğuna işaret etmektedir. Atak sırasında ve remisyonda ölçülen düzeyler arasındaki farkın istatistiksel anlamlılığa ulaşmaması hasta grubumuzun küçük olması ile ilişkili olabilir.
