Brachial amyotrophic diplegia: Case report

Abstract

Two forms of Amyotrophic lateral sclerosis (ALS) subtypes have been recognized since the late 19th and early 20th centuries but relatively inadequately studied, these being the flail arm (FA) and flail leg (FL) syndromes. The FA phenotype was described by Vulpian in 1886 as a syndrome of proximal weakness and wasting of the upper limbs (scapulohumeral variant of progressive muscular atrophy or forme scapulo-hume ́rale). The condition has been variously termed as Vulpian-Bernhardt syndrome, hanging-arm syndrome, neurogenic man-in-a-barrel syndrome, brachial amyotrophic diplegia, or the FA syndrome. The syndrome typically presents with progressive upper limb weakness and wasting that is often symmetric and proximal, without significant functional involvement of lower limbs or bulbar muscles. Here we presented a patient with complaints of difficulty in lifting his right arm in his medical history. Brachial amyotrophic diplegia was diagnosed with neurological examination and EMG findings. It is presented because of rarity.

19. yy sonu 20. yy başlarında flail arm (FA), flail leg (FL) sendromu olarak isimlendirilen Amyotrofik lateral sklerozis (ALS)'nin 2 alt grubu tanımlanmıştır. FA fenotipi Vulpian tarafından 1886'da üst ekstremitenin proksimal güçsüzlüğü ve atrofisi sendromu olarak (Progressif müsküler atrofi'nin skapulohümoral varyantı) tanımlanmıştır. Bu durum sonraki yıllarda Vulpian-Bernhardt sendromu, hanging-arm sendromu, nörojenik man-in-a-barrel sendromu, brakiyal amyotrofik dipleji ya da FA sendromu olarak farklı isimlerle adlandırılmıştır. Bu sendrom tipik olarak progresif, yavaş seyirli üst ekstremite güçsüzlüğü, zayıflığı ile seyreden alt ekstremite ve bulber kaslarda fonksiyonel zaafiyete neden olmayan sıklıkla simetrik proksimal bir hastalıktır. Burada sağ kolunu kaldırma güçlüğü şikayeti ile gelen anamnez, nörolojik muayene ve EMG ile brakiyal amyotrofik dipleji tanısı konan bir olgu nadir görülmesi nedeniyle sunulmuştur.
