Prognosis and Clinical Features of Idiopathic Generalised Epilepsy Patients Older Than 40 Years

Abstract
Introduction: Idiopathic generalised epilepsies (IGE) are usually seen in the first twodecades of life. There is a paucity of data of these patients when they get older.Methods: Patients admitted to our epilepsy center were screened for being older than 40years at the time of last admission and being diagnosed with IGE according to ILAE-1989criteria. The clinical features, syndromes and prognosis of 33 patients meeting the inclusioncriteria were evaluated retrospectively.Results: Nineteen (58%) were female and mean age was 50,24±10,29. 16 cases had juvenilemyoclonic epilepsy, 7 juvenile absence epilepsy, 4 childhood absence epilepsy, 2 IGE withgeneralised convulsive seizures (GCS), 3 atypical IGE with absence status epilepticus (ASE),1 late onset IGE. 19 patients were using monotherapy whereas 13 were using polytherapy,and there is only one case without medication. The prognosis was evaluated as good in 54.5%(n:18) and as poor in 45.5 % (n: 15). Absence of family history of epilepsy, male gender wererelated with good prognosis without statistical significance. Prognosis was observed as betterin the co-existence of myoclonia and GCS and in the group with GCS only, whereas theexistence of absence seizures and frequent ASE were associated with poor prognosis.Conclusions: Almost half of the IGE patients older than 40 years had poor prognosis andshould be followed up closely. It is remarkable that this group consists of well-known IGEsyndromes diagnosed earlier rather than being late onset and also includes atypical patientsexperiencing frequent ASE.

Giriş: İdyopatik jeneralize epilepsiler (İJE) genellikle hayatın ilk 20 yılında görülürler. Buhastaların ileri yaşlarda durumuna ilişkin bilgi çok azdır.Metod: Epilepsi merkezimize başvurmuş tüm olgular taranmış ve son başvuru tarihine göre40 yaşında ve üzerindeki, ILAE 1989 kriterlerine göre İJE tanısı almış olgular araştırılmıştır.Çalışmaya 33 hasta alınmıştır. Hastaların klinik ve laboratuar özellikleri, sendrom dağılımlarıve prognoz özellikleri retrospektif olarak değerlendirilmiştir.Bulgular: Olguların 19'u (%58) kadındı ve ortalama yaş 50,24±10,29 idi. Onaltı olgudaJüvenil Miyoklonik Epilepsi (JME), 7 olguda Jüvenil absans epilepsisi (JAE), 4 olgudaÇocukluk Çağı Absans Epilepsisi (ÇAE), 2 olguda jeneralize konvülzif nöbetleri olan İJE, 3olguda absans status epileptikus (ASE) ile seyreden atipik İJE, 1 olguda geç başlangıçlı İJEvardı. Ondokuz hastanın son tedavisi monoterapi, 13 hastanın tedavisi politerapi şeklindeydi,1 hasta ilaç kullanmıyordu. Prognoz açısından %54.5 (n:18) hasta iyi , %45.5 (n: 15) hastakötü prognozlu olarak değerlendirildi. Ailede epilepsi öyküsü olmaması ve erkek cinsiyet ileiyi prognoz arasında istatistiksel olarak anlamlı olmamakla birlikte ilişki olduğu görüldü.Miyokloni ve jeneralize konvülzif (JK) nöbeti birlikte olanlar veya sadece JK nöbeti olanlardaprognozun görece daha iyi olduğu saptanırken absans nöbet varlığının ve sık ASE ileseyreden İJE'lerin ileri yaşta kötü prognozla ilişkili olduğu gözlendi.Sonuç: Kırk yaş üzerindeki İJE'li hasta grubu bilinenin aksine, epilepsi merkezimizde yarıyayakın oranda kötü prognoz ile ilişkili ve düzenli takip edilmesi gereken hasta grubunuoluşturmaktadır. Geç başlangıçlı olmaktan çok nöbetleri devam eden bilinen sendromluolgulardan oluşan bu grupta ASE ile seyreden atipik olguların varlığı da dikkat çekicidir.
