Abstract

A 79-year-old diabetic woman with a fresh T9 osteoporotic compression fracture, underwentpercutaneous kyphoplasty using polymethyl methacrylate. About 2.5 months after theoperation, the patient was readmitted because of increasing back pain, nocturnal sweating andsubfebrile temperature. The diagnosis of pyogenic spondylitis was made using radiograph andmagnetic resonance imaging. The patient was successfully treated with antibiotics for 2month. Only five cases of infections after kyphoplasty have been described so far. Our case isprobably the first post kyphoplasty spondylitis which was treated non-operatively. For thekyphoplasty patients with diabetes and older age, specific modifications to prophylacticantibiotic protocols such as using higher doses of cefazolin or broad-spectrum antibioticsshould be considered.

Özet

Yeni gelişmiş osteoporotik T9 çökme kırıklı 79 yaşındaki bir kadına polimetil metakrilatkullanılarak perkütan kifoplasti uygulandı. Operasyondan yaklaşık 2.5 ay sonra hastasubfebril ateş, gece terlemesi ve sırt ağrısında artış nedeni ile yeniden başvurdu. Radyografive manyetik rozenans görüntüleme kullanılarak pyojenik spondilit tanısı kondu. Hasta 2 ayantibiyotik verilerek başarılı bir şekilde tedavi edildi. Şimdiye kadar kifoplasti sonrasındayanlızca beş enfeksiyon olgusu tanımlanmıştır. Bizim olgumuz muhtemelen non-operatifolarak tedavi edilen ilk post kifoplasti spondilitidir. Yaşlı ve diyabetik kifoplasti hastaları için,proflaktik antibiyotik protokollerinde, yüksek dozda sefazolin veya geniş spekturumluantibiyotik kullanmak gibi spesifik modifikasyonlar değerlendirilmelidir.
