Multipl Hemorajik Metastatik Koriokarsinom ile İntraserebral Hemorajide Yeniden Kanama

Early Rebleeding of Multiple Hemorrhagic Metastatic Choriocarcinoma with Intracerebral Hemorrhage

Abstract

Choriocarcinoma is a rare, highly malignant gestational trophoblastic neoplasia. This tumor isknown for its rapid hematogenous spread to multiple organs and good response tochemotherapy. However, cerebral metastases occur frequently, and hemorrhage from cerebralmetastases is a common cause of death. Therefore, patients with cerebral metastases requireintensive multimodal therapy to achieve remission. Here, we report the case of a 38-year-oldwoman with multiple hemorrhagic metastases of choriocarcinoma who developedintracerebral hemorrhage, for which she underwent craniotomy and total removal of thehematoma. Five days after surgery, rebleeding of a large intracerebral hemorrhage developedand an emergency reoperation was performed. We suggest that surgeons carefully consider allpatient-related data and always pay attention to changes in the neurological state of a patient.Surgeons should also do their best to completely remove the intracerebral choriocarcinoma inthe first operation.

Özet

Koriyokarsinom ender görülen yüksek derecede habis gestasyonal trofoblastik birneoplazmadır. Bu tümör bir çok organa hızlı kan yayılımı ve kemoterapiye iyi yanıt vermesiile tanınır. Bununla birlikte beyin metastazları oldukça sık görülür ve bu metastatiklezyonlarda kanama en yaygın ölüm nedenidir. Bu nedenle remisyona ulaşmak üzere serebralmetastazları olan hastalar daha yoğun multimodal tedaviye gereksinim duyarlar. Bu makaledemultipl hemorajik metastatik koriyokarsinomu olan ve kraniotomi ile hematomun tümü eksizeedilen 38 yaşında bir kadın olgu sunuyoruz. Cerrahiden beş gün sonra geniş bir intraserebralkanama nedeniyle ivedi bir ameliyat daha yapılmak zorunda kalındı. Önerimiz hastaya bağlınörolojik değişikliklerin dikkatlice değerlendirilmesi ve cerrahların intraserebralkoriyokarsinomun ilk müdahalesi sırasında total çıkartılması için ellerinden geleninazamisinin yapmalarıdır.
