Bilateral Intracranial Subacute Subdural Hematoma After Lumbar Epidural Steroid Injection: Case Report

Abstract
Epidural steroid administration is a frequently used method for the treatment of chronic backpain. Major complications range from transient headache and pain on injection site tohemorrhage. Dural puncture occured while injecting the second epidural steroid dosage to a24-year-old male patient with lumbar disc hernia. After 6 days, the patient was hospitalizeddue to persistent headache; he was discharged with appropriate recommendations when hisheadache resolved after 6 days of treatment. However, he presented to the pain clinic withheadache, nausea and vomiting on the 13. day of discharge (i.e. 25 days after the secondepidural injection). MR imaging revealed hemorrhagic subdural effusion and diffuse duralthickening. Hematoma was evacuated surgically and the patient was discharged on the 5.postoperative day without neurological deficits. Eventhough the first explanation that comesto our mind is postdural puncture for headache occuring after epidural applications, it shouldnot be forgotten that one might encounter rare life-threatening complications like subduralhematomas. In our patient who did not have neurological deficits and was thought to havepostdural puncture headache, the change and increase in symptoms led us to considerintracranial hemorrhage. Our patient was diagnosed in the subacute period, treatedappropriately and discharged without neurological deficits. To our knowledge, this the firstcase reported in the literature for bilateral subdural hematoma occuring after epidural steroidinjection. Patients presenting with headache after dural puncture should be followed carefullyfor intracranial hemorrhage, and further examinations carried out when judged necessary.

Epidural steroid uygulamaları, kronik sırt ağrılarının tedavisinde sıklıkla kullanılanyöntemlerdendir. Major komplikasyonları; geçici baş ağrısı, injeksiyon yerinde ağrıdankanamaya kadar dağılım göstermektedir. Lomber disk hernisi olan 24 yaşında erkek hastaya2. doz epidural steroid enjeksiyonu uygulanırken dura ponksiyonu gerçekleşti. 6 gün sonrabaş ağrısı nedeniyle hospitalize edilen hasta 6 günlük tedavi sonrası baş ağrısı geçinceönerilerle taburcu edildi. Taburculuğundan 13 gün, 2. epidural girişiminden 25 gün sonra başağrısı, bulantı-kusma şikayetleriyle ağrı polikliniğimize tekrar başvurdu. Çekilen MR'dahemorajik vasıfta subdural effüzyon, tüm durada kalınlaşma izlendi. Hematom cerrahi olarakboşaltıldı. Hasta postop 5. günde nörolojik defisiti olmadan taburcu edildi. Epidural girişimsonrası gelişen baş ağrısında ilk akla gelen post dural ponksiyon baş ağrısı olsa da subduralhematom gibi hayatı tehdit eden nadir bir komplikasyonda unutulmamalıdır. Erken dönemdepost dural ponksiyon baş ağrısı düşündüğümüz, nörolojik defisiti olmayan hastamızda,semptomların değişmesi ve şiddetlenmesi intrakraniyal kanamayı aklımıza getirdi. Subakutdönemde tanısı konan ve tedavi edilen, nörolojik defisit gelişmeden şifa ile taburcu olanolgumuz literatür taramalarımıza göre epidural steroid enjeksiyonu sonrası bilateral subakutsubdural hematom gelişen ilk vakadır. Dura ponksiyonu sonrası baş ağrısı gelişen tümhastalar intrakranial hemoraji açısından da izlenmeli gerekli görüldüğünde ileri tetkiklerebaşvurulmalıdır.
