L3 Osteosarkomuna Total Spondilektomi Cerrahisi

Abstract

Primary osteosarcoma is a rare malignant osseous tumor of vertebra. Prognosis has beenimproved with new technologies and surgical techniques. In modern era of spinal surgery enbloc resection has become more popular in these neoplasms. We presented a 19-year-oldyoung man admitted to our clinic with low back pain. An L3 vertebral mass lesion wasdetected on MRI. CT-guided biopsy result was osteosarcoma. En bloc resection was planned.In the literature, intraoperative blood loss amounts were very high in osteosarcoma cases, sowe decided to embolize the tumor pre-operatively. It worked in the surgery that only 700 mlof blood was lost. Although surgery was one staged (combined anterior and posteriorapproaches), it took 6 hours with 2 surgeons. Definitive diagnosis was osteosarcoma and thepatient was referred to oncologists. Embolization should be kept in mind before vertebraltumor surgery to make it smooth and easy.

Özet

Primer osteosarkom vertebranın nadir kemik tümörüdür. Gelişen teknoloji ve cerrahitekniklerle hastalığın prognozu daha iyiye gitmektedir. Modern omurga cerrahisinde, bu türtümörlerde en blok rezeksiyon popüler hale geldi. Kliniğimize, 19 yaşında genç bir erkekhasta bel ağrısı ile başvurdu. MR görüntülemede L3 vertebrada kitle tespit edildi. BT- görüntülü biyopsi işlemi sonucunda osteosarkom tanısı konuldu. En blok rezeksiyonplanlandı. Literatürde, omurga osteosarkom olgularının cerrahisinde, cerrahi içi kan kaybınınyüksek olması sebebiyle, kendi olgumuzda ameliyat öncesi, tümöre yönelik embolizasyonişlemi uygulandı. Böylece hastanın ameliyatı esnasında toplam kan kaybı 700 ml ilesınırlandı. Ayrıca, her ne kadar kombine cerrahi (ön ve arka yaklaşımın beraber olduğu)uygulanmış olsa da, iki uzman cerrahın katıldığı işlem toplam 6 saat sürdü. Kesin tanıosteosarkom geldi ve hasta onkolojiye yönlendirildi. Omurga tümör cerrahisinde, ameliyatöncesi embolizasyon işlemi akılda bulundurulmalıdır.
