Abstract

Subduroperitoneal shunting is a commonly used treatment option for subdural fluidcollections. Cranial migration of subduroperitoneal shunt is a rare complication. We reporttwo cases with cranial migration of subduroperitoneal shunts. Both of them had a previouscranial surgery and they secondarily developed subdural fluid collections. These collectionswere treated with subduroperitoneal shunting using a single peritoneal catheter. Inadequatefixing can result in migration of these single catheters. Elevated intra-abdominal pressure, dueto any occasion may also help this migration. We recommend using silk or other braided non- absorbable sutures and anchoring wings for fixing single catheters when used in treatment ofsubdural fluid collections.

Özet

Subduroperitonal şant takılması, subdural sıvı koleksiyonları için sıkça kullanılan bir tedaviseçeneğidir. Subduroperitoneal şant kateterinin kranyal migrasyonu nadir birkomplikasyondur. Subduroperitoneal şantları kranyal migrasyon gösteren iki olgubildirmekteyiz. Iki hasta da daha önce kranyal cerrahi geçirmişlerdi ve ikincil olarak subduralsıvı koleksiyonu geliştirmişlerdi. Iki hastada da bu subdural koleksiyonlar tek parça birkateterin kullanıldığı subduroperitoneal şant takılarak tedavi edildi. Bu tek parça kateterlerinuygun olmayan teknikle tespit edilmesi migrasyona neden olabilir. Herhangi bir nedenleartmış intraabdominal basınç ta bu migrasyona katkıda bulunabilir. Biz subdural sıvıkoleksiyonunu tedavi etmek üzere kullanılacak tek parça subduroperitoneal şant kateterlerininipek veya diğer örgülü emilebilir olmayan sütürler ve yardımcı tespit materyalleri ile(anchoring wings) tespit edilmesini önermekteyiz.
