Evaluation of The Etiology of Man in The Barrel Syndrome With Four Cases

Abstract
Man-in-the barrel syndrome is a descriptive term of proximal dominant weakness for upperlimb and shoulder girdle muscles. The patient is constrained in a barrel around the trunk,completely prohibiting upper limb movements. The most common cause of this syndrome isbilateral anterior watershed infarctions due to cerebral hypoperfusion of anterior and middlecerebral artery territory. Furthermore, polyneuropathies, motor neuron diseases, centralpontine myelinolysis and cervical spinal cord lesions lead to this clinical presentation.Because of unfavorable prognosis of this syndrome in most cases, the treatable etiologicaldetermination of the causes is crucial.The present paper discusses the etiology of 'Man in the barrel syndrome'' and investigationsinto differential diagnosis accompanied by four cases who have bilateral proximal upper limbweakness.
Özet
Fıçıdaki Adam Sendromu üst ekstremite ve omuz kuşağı kaslarında proksimalde egemengüçsüzlük tablosudur. Hasta üst ekstremitesini hareket ettiremediği bir fıçı içinde gibidir. Busendromun en sık nedeni serebral hipoperfüzyona bağlı orta ve anterior serebral arter vaskülersulama alanlarındaki watershed infarktlarıdır. Bunun yanında polinöropatiler, motor nöronhastalıkları, santral pontin myelinozis ve servikal spinal lezyonlar da bu klinik görünümeneden olmaktadır. Bu sendromun prognozu birçok olguda iyi seyirli olmadığından, tedaviedilebilir etyolojik nedenlerin belirlenmesi önemli bir yer tutmaktadır.Burada her iki kol proksimal kaslarında güçsüzlük şikayeti ile başvuran dört olgu eşliğinde,Fıçıdaki adam sendromunun etyolojisinde yer alan hastalıkları ve ayırıcı tanı için yapılanincelemeleri tartıştık.
