Creutzfeldt-Jakob Hastalığı-Heidenhain Varyantı: EEG ve Kraniyal MRI Bulguları

Abstract
Creutzfeldt Jakob disease (CJD) is the most common prion disease. The Heidenhain variant of CJD'sclinical feature is the association with visual symptoms such as progressive visual decline, corticalblindness, visual agnosia and visual hallucinations. Case: Sixty-year old man presented to us withprogressive dementia, gait disturbances, audio-visual hallucinations and behavioral changes. On hisneurological examination, the patient was confused, his attention span was decreased andaccompanied by disorientation to time and place. There was tetraparesis and deep tendon reflexes wereabsent. The Babinski's sign was bilaterally negative. The myoclonic jerks are induced tactile orauditory stimulus. His cranial magnetic resonance imaging (cMRI) and diffusion-weighted imaging(DWI) showed cerebral and cerebellar atrophy, T2 and FLAIR-weighted sequences of basal ganglia,thalamus and occipital cortex showed increased signal intensity and diffusion limitation. Hiselectroencephalography (EEG) findings showed low-amplitude slow wave activity in bothhemispheres and periodic sharply contoured slow-wave in the occipital regions. The cerebrospinalfluid's (CSF) protein was high level and the result of CSF protein 14-3-3 was negative. The patientdied in 6th month of clinical follow-up. Conclusion: The Heidenhain variant of CJD should beconsidered in all patients who present with dementia, ataxia and visual phenomena. Close follow-up aswell as serial EEG can help clarify the underlying disease process. And DWI and FLAIR sequencesshould be included in the cMRI protocol.

Creutzfeldt Jakob hastalığı (CJH), en sık görülen prion hastalığıdır. CJH-Heidenhain varyantı,eşlik eden görsel semptomlarla (progresif görme kaybı, kortikal körlük, vizüel agnozi vegörsel halüsinasyonlar) karakterizedir. Olgu: Altmış yaşında erkek hasta, progresifunutkanlık, davranış değişikliği, yürüme bozukluğu, işitsel ve görsel halüsinasyonlar ilebaşvurdu. Nörolojik muayenesinde, bilinci konfuze idi. Kooperasyonu kısıtlı, oryantasyonubozuktu. Motor system muayenesinde tetraparezi mevcuttu ve derin tendon reflekslerialınamadı. Babinski işareti iki yanlı negatifti. İşitsel ve görsel uyaranla tüm vucuttamyoklonik atmaları vardı. Kraniyal manyetik rezonans (cMRI) ve difüzyon MR'da (DWI)serebral ve sereballar atrofi, T2 ve FLAIR sekanslarda bazal ganglion, talamus ve oksipitalkortekste sinyal artışı vardı. EEG incelemesinde, düşük amplitüdlü, yavaş dalga aktivitesi veher iki oksipital bölgede periodik özellik gösteren, keskin konturlu yavaş dalga paroksizmleriizlendi. Beyin omurilik sıvısında, protein düzeyi yüksekti ve 14-3-3 proteini negatifti. Klinik izlemin 6. ayında hasta kaybedildi. Tartışma: CJH-Heidenhain varyantı, demans, ataksi vevizüel semptomlarla başvuran hastalarda akla getirilmelidir. Klinik izlemde, seri EEG kaydıve cMR inceleme protokolüne DWI ve FLAIR sekanslarının eklenmesi ihmal edilmemelidir.
