Simultaneous Expansion of Pseudomeningocele and Development of Remote Subdural Hygroma: A New Phenomenon

Abstract

Development of subdural hygroma (SDG) distant from primary neurosurgery is uncommon.We present a rare relationship between remote supratentorial extension of a local SDG andexpansion of an infratentorial pseudomeningocele following resection of a cerebellarhemangioblastoma in a 61-year-old female with von Hippel-Lindau disease. Initial Magneticresonance imaging showed a cystic hemangioblastoma in the right cerebellum with masseffect on the brain stem. The patient undertook median suboccipital craniectomy withduraplasty followed by development of bilateral retrocerebellar SDG and apseudomeningocele beneath the dural graft. Nineteen days after the operation, a remote SDGappeared supratentorially with concurrent interval growth of the pseudomeningocele. Earlymanagement of the growing pseudomeningocele may minimize the occurrence of remoteSDG as a post-operative complication.

Primer cerrahi yerinden farklı bir yerde subdural higroma (SDG) gelişmesi yaygın değildir.Von Hippel- Lindau Hastalığına bağlı serebellar bir hemangioblastomu olan 61 yaşında birkadın hastada gelişen infratentoriyel psödomeningosel ile eşzamanlı olarak supratentoriyeluzanım gösteren ender lokal bir subdural higroma ilişkisini olgu sunumu olarak sunmaktayız.MR tetkiki pre-op olarak sağ serebellum hemisferinde beyin sapına kitle etkisi gösteren kistikbir hemangioblastomu göstermekte idi. Hasta orta-hat suboksipital kraniektomi ve duraplastioperasyonu geçirdi. Hastada postoperatif olarak bilateral retroserebellar SDG ve duralyamanın altında psödomeningosel gelişti. Ameliyattan 19 gün sonra psödomeningoselinbüyümesi ile eşzamanlı supratentoryel olarak uzak yerleşimli bir SDG belirlendi. Büyüyenpsödomeningoselin erken tedavisi bir post-operatif komplikasyon olan eşzamanlı uzakgelişebilecek bir SDG.
