Kifoplastide Komşu Diskten Epidural Sement Kaçağı: Bir Olgu Sunumu

Abstract

A 56-year-old woman with a T8 fracture, underwent percutaneous kyphoplasty usingpolymethyl methacrylate. An epidural cement leakage had occurred through the T7-T8 discwithout the cement reaching the posterior part of the T8 vertebral body. This leakage did notcause any neurological deficit or any complaint. The patient's complaint of back pain wasresolved and three days after the kyphoplasty she was discharged, without any neurologicdeficit. The described leakages into the spinal canal by now, had occurred through fractures ordeficiencies in the vertebral cortex or by injection of cement into the basivertebral vein. Ourcase showed that epidural cement leakage can occur from the adjacent discs through a radialannular tear as well as from the fractured posterior wall of the vertebral body directly.Physician should remain aware about such complications.

Özet

56 yaşındaki T8 kırıklı bir kadına polimetil metakrilat kullanılarak kifoplasti uygulandı.Sement T8 vertebra gövdesinde posterior bölüme ulaşmadan T7-T8 diskinden epiduralsement kaçağı oldu. Bu kaçak herhangi bir nörolojik defisit veya herhangi bir şikayete nedenolmadı. Hastanın sırt ağrısı düzeldi ve kifoplastiden üç gün sonra herhangi bir nörolojikdefisit olmaksızın taburcu edildi. Şimdiye kadar spinal kanala olduğu bildirilen kaçaklarvertebral korteksteki kırık veya yetmezliklerden, ya da basivertebral vene sement enjeksiyonuile oluşmuştur. Bizim olgumuz epidural sement kaçağının vertebral gövdenin posteriorduvarındaki bir fraktürden direkt olarak olmasının yanısıra komşu diskteki radial annuler biryırtık yoluyla da olabileceğini göstermiştir. Hekimler bu tip komplikasyonların farkındaolmalıdırlar.
