Epilepsi Hastalarında Sıklık Alternan Pattern Parametrelerinde Ortaya Çıkan Değişiklikler

Abstract
Aim: The aim of this study is to determine the changes in the polysomnographic parametersand the cyclic alternating pattern (CAP) in generalized and partial epilepsy patients (with andwithout epileptiform discharges in EEG) using video-EEG-PSG recording.Materials and Methods: 73 patients diagnosed with epilepsy and 19 healthy controls withinthe same age group (control group) underwent an 8 hour long sleep video-EEG-PSGrecording. After the first evaluation, CAP parameters were scored in 57 patients (31generalized and 26 partial epilepsy) and 16 healty subjects who have no sleep diseases and theresults were compared within groups.Results: Total sleep time and NREM I phase were found to be longest in the partial epilepsygroup, shortest in the control group while REM phase was just the opposite to this. MeanCAP ratios were found to be statistically higher in the generalized epilepsy group whencompared to other two groups. This difference was found to be continued when the controlgroup and the generalized epileptic patients who had no abnormality in EEG. No differencewas found between the partial epilepsy and control group regarding CAP ratios.Conclusion: Patients with generalized epilepsy have differences compared to healthyindividuals regarding the macro and microstructure of sleep and it seems that thesedifferences are independent of the epileptiform discharges. In partial epilepsy patients, nomicrostructural differences were detected while macrostructural changes were evident.

Amaç: Bu çalışmanın amacı jeneralize ve parsiyel epilepsili (EEG'de epileptiform deşarjlarıolan ve olmayan) hastalarda video-EEG-PSG kayıtlaması ile polisomnografik parametreler vesiklik alternan pattern (CAP) değişikliklerinin araştırılmasıdır.Yöntem: Epilepsi tanılı 73 hasta ve aynı yaş grubunda 19 sağlıklı olguya (kontrol grubu) 8saatlik uyku video-EEG-PSG kayıtlaması yapıldı. İlk değerlendirme sonrasında, uykubozukluğu saptanmayan 57 hasta (31 jeneralize ve 26 parsiyel epilepsi tanılı) ve 16 sağlıklıolgunun kayıtlarında CAP parametreleri skorlandı ve sonuçlar karşılaştırıldı.Bulgular: Total uyku zamanı ve NREM I fazı parsiyel epilepsi grubunda en uzun, kontrolgrubunda en kısa; REM fazı ise parsiyel epilepsi grubunda en kısa, kontrol grubunda en uzunsaptandı. Ortalama CAP oranları jeneralize epilepsi grubunda diğer 2 gruba göre anlamlıyüksek saptandı ve bu fark EEG anormalliği olmayan jeneralize epileptik hastalar ile kontrolgrubu arasında devamlılık gösterdi. Parsiyel epilepsi hasta grubu ve kontrol grubu arasındaCAP oranı karşılaştırıldığında fark saptanmadı.Sonuç: Jeneralize epileptik hastalarda uykunun makro ve mikroyapısında sağlıklı kişileregöre değişiklikler ortaya çıkmaktadır ve bu değişiklikler epileptiform deşarjlardan bağımsızgörünmektedir. Parsiyel epilepsi hastalarında ise makroyapısal değişiklikler belirgin ikenmikroyapısal değişiklik gözlenmemiştir.
