Abstract

Aim: The aim of this study is to analyse the results of patients diagnosedwho had also undergo surgery for total hip artroplasty. Material and Method:During the study, clinical and radiological evaluations were performed for75 hips of 72 patients in Orthopedics and Traumatology clinic between theyears of 2002 and 2008. The 52% of the patients were women (38); while48% of them were men (34) and 55.5% was the left side (40), whereas 40.2%was the right side (29) and 5.5% was bilateral (4). The age distribution wasin the range of 26-88 (average 64.3). A total of 75 femoral componentswere placed in the patients. 10 (13.3%) of these components were cemented;whereas 65 (86.7%) of them were cementless. A total of 75 acetabular com- ponents were placed in the patients. 66 (88%) of these components werescrew acetabular components; whereas 9 (12%) of them were acetabularcomponents without screws (porous coated). For the clinical evaluation ofthe patients, Harris Hip Evaluation Score and Visual Analogue Scale (VAS)were used. Results: The average Harris Hip Evaluation score was 36.7 beforethe operation, while it was 70.3 in the recent control; and the preoperationalaverage VAS was 81.6 whereas the average VAS observed to be 35.7 in thelast control. Discussion: As a consequence the results of total hip artroplastyare observed to be excellent due to accurate indication, suitable implantselection, proper surgical technique proper postoperative maintenance andrehabilitation. In particular, total hip artroplasty takes a significant place forthe relief of the pain that limits the life and can not be reduced, generallyobserved in patients who are 65 years old and above; and which can not berelieved by conservative methods for which the only alternative is hip jointresection.

Amaç: Bu çalışma ile total kalça protezi endikasyonu konulan hastalara uygulanan total kalça artroplastisi ameliyatlarının sonuçları araştırılmıştır. Gereç ve Yöntem: Kliniğimizde 2002 2008 yılları arasında 72 hastanın 75 kalçası klinik ve radyolojik olarak değerlendirildi. Hastalardan 38i kadın (% 52), 34ü erkek (% 48), 40ı (% 55.5) sol taraf, 29u (% 40.2) sağ taraf ve 4 de (% 5.5 bilateral idi. Hastaların yaş dağılımı 26 88 (ort 64.3) arasında idi. Hastalara toplam 75 femoral komponent konuldu. Bu komponentlerin 10u (% 13.3) çimentosuz olarak, 65i (% 86.7) ise çimentolu olarak femura yerleştirildi. Hastalara toplam 75 asetabuler komponent konuldu. Bu komponentlerin 66ı (% 88) vidalı asetabular komponent, 9u (% 12) vidasız asetabuler komponent (poroz kaplı) idi. Hastaların klinik değerlendirmesinde Harris Kalça Degerlendirme Skoru ve Vizüel Analog Skalası (VAS) kullanıldı. Bulgular: Operasyon öncesi ortalama 36.7 olan Harris Kalça Değerlendirme Skoru en son kontrolde 70.3, operasyon öncesi VAS ortalama 81.6 olup, en son kontrolde VAS ortalama 35.7 olduğu görüldü. Tartışma: Total kalça protezinin (TKP); doğru endikasyon, doğru implant seçimi, doğru cerrahi teknik, doğru postoperatif bakım ve rehabilitasyon sayesinde sonuçlarının çok iyi olduğunu görmekteyiz. Özellikle hayatı sınırlayan, 65 ve üzeri yaş grubunda görülen ve konservatif metodlarla giderilemeyen, tek cerrahi alternatifi kalça eklemi rezeksiyonu olan ağrının giderilmesinde TKPnin çok önemli bir yeri vardır.
