Abstract

Aim: In this study, we aimed to compare the effects of propofol and desflurananesthesia combined with remifentanil anesthesia on the various systemsin short-term breast surgery. Material and Method: A prospective, randomized, controlled study design established for the short-term breast surgeryplanned, 40, ASA I- II patients aged 18-60 year-old. Group P (n=20) wasnamed for the patients administered remifentanil and propofol and Group D(n=20) was named for the patients administered remifentanil and desflurane.For induction therapy, remifentanil of 1µg/kg dosage and Propofol of 2mg/kgdosage was adjusted for Group P and remifentanil of 1µg/kg dosage and %3 end expiratory desflurane was adjusted for Group D. LMA applied for all ofthe patients. Maintenance treatment of remifentanil infusion with the doseof 0,25 µg/kg/min were used for both groups and Group P was also administered to 90 µg/kg/min infusion of Propofol. Tenoksikam with the dose of20mg was used intravenously for all the patients 10min before the surgeryfinalized. Data of operation time, total anesthesia time, LMA extubation time,eye opening time, the time achievement of Aldrete score of 9 in postoperative period, VAS results and total remifentanil consumption were recorded.Blood samples were collected for ACTH and Cortizol levels 10min. beforeinduction, 15 min after initial surgical incision and at 15th min of recoveryperiod. Results: Hemodynamic stability maintained for both groups and nohemodynamic response occurred after surgical incisions. Wake-up time andrecovery time were decreased in group D. We detected that ACTH and Cortizol levels were decreased in both groups although surgical stress was increased. Discussion: We suggest that desflurane-remifentanil drug combination can be used successfully as an alternative to TIVA for outpatient surgery.

Özet

Amaç: Bu çalışmada kısa süreli meme cerrahisinde remifentanil ile kombine edilen propofol ve desfluran anestezisinin çeşitli sistemler üzerine olan etkilerinin karşılaştırılması amaçlandı. Gereç ve Yöntem: Kısa süreli meme cerrahisi planlanan ASA I- II, 1860 yaş arası 40 hasta prospektif, randomize kontrollü klinik çalışmaya alındı. P grubuna (n=20) remifentanil ve propofol D grubuna (n=20) remifentanil ve desfluran anestezisi uygulandı. İndüksiyoniçin P grubuna remifentanil 1 µg/kg ve propofol 2 mg/kg, D grubuna ise remifentanil 1 µg/kg ve % 3 end ekspiratuar olacak şekilde desfluran kullanıldıktan sonra hastalara LMA yerleştirildi. İdamede her iki gruba da 0,25 µg/ kg/dkdan remifentanil infüzyonu ve P grubuna ilave olarak 90 µg/kg/dk propofol infüzyonu uygulandı. Tüm hastalara cerrahi bitimine 10 dk. kala 20 mg Tenoksikam iv. olarak yapıldı. Operasyon zamanı, anestezi süresi, LMA çıkarılma zamanı, göz açma zamanı, postoperatif Aldrete derlenme skoru 9 olma zamanı, VAS değerleri ve toplam remifentanil tüketim miktarları tespit edildi İndüksiyondan 10 dk. önce, cerrahi insizyondan 15 dk. sonra ve derlenmenin 15. dk.sında ACTH ve Kortizol ölçümleri için kan alındı. Bulgular: Her iki grup hastalarda da hemodinamik kontrolün iyi olduğu ve cerrahi insizyona hemo- dinamik yanıt gözlenmediği, grup D de uyanma ve derlenme sürelerinin daha kısa olduğu, her iki grup ACTH ve Kortizol değerlerinin cerrahi strese rağmen azaldığı tespit edildi. Tartışma: Günübirlik cerrahide TİVAya bir alternatif olarak desfluranremifentanil anestezisinin de başarılı bir şekilde kullanılabileceği düşüncesindeyiz.
