Protez Diş Aspirasyonu Sonrası Pnömoni Gelişen Erişkin Bir Hasta

Abstract

In patients with a new and suddenly beginning of bronchial asthma and repetitive respiratory infection, one should consider the possibility of obstruction by aforeign body. In this case we present a male patient with pneumonia due to aspi- ration of dental prosthesis. A 32-year-old man presented with a 4-month historyof dyspnea, chest pain and newcompliants of diaphoresis and fever persistenddespite of antibiotic treatment. Then the patient consultated to our chest surgerydepartment. After the physical examination of patient we made the fiberopticflexible bronchoscopy according to the uncertain opasity seen in the chest radi- ography. Later in the operating room, with the patient under general anesthesia,we extracted the aspirated foreign body, dental prosthesis, by rigid bronchoscopy.We should think about the possibility of foreign body aspiration for the patientswith suddenly beginning of the complaints such as, bronchial asthma, repetitiverespiratory infection, diaphoresis, chest pain and fever. Sometimes people hidesthe true anamnesis because of be ashamed or amnesia.

Özet

Yeni ve ani başlayan bronşial astım benzeri semptomlar ve sık tekrarlayan enfeksiyon atakları olan hastalarda yabancı cisim aspirasyonu ayırıcı tanıda düşünülmelidir. Bu olgu sunumunda diş protezi aspirasyonuna sekonder gelişen pnömonili birhastayı sunmayı amaçladık. Otuziki yaşında erkek hasta ani gelişen ve 4 ay sürennefes darlığı, göğüs ağrısı ve son zamanlarda ortaya çıkan ateş ve terleme şikayetleri, aldığı antibiyoterapi ve bronkodilatatör tedaviyle geçmemesi üzerine göğüs cerrahisi bölümüne konsültasyon için yönlendirilmişti. Hastanın fizik muayenesi yapılıp, direkt akciğer grafisi görüldükten sonra ön tanımızı desteklemek amaçlı yapılan fiberoptik bronkoskopi tetkiki sırasında sağ ana bronşta yabancı cisimsaptandı. Genel anestezi altında yapılan rijit bronkoskopi ile yabancı cisim olarakdiş protez parçası çıkarıldı. Ani gelişen nefes darlığı, ateş, terleme ve göğüs ağrısı şikâyetleriyle gelen hastada ayırıcı tanıda yabancı cisim anamnezi derinleştirilmeli. Bazı durumlarda hastanın unutkanlık veya utangaçlık gibi nedenlerle yetersizanamnez vermesi ile tanı atlanabilinmektedir.
