The relationship between the expression of cyclin D1, c-erbB-2, apoptolic index and the depth of myometrial invasion, grade, survival rate in stage I endometrial cancer

Abstract
OBJECTIVE: Cyclins are a group of proteins that are involved in the control of the entry and progression of proliferating cells through the key checkpoints in the cell cycle. Cyclin D1 protein plays role in the transition of G1-S phase and its overexpression is thought to be related in neoplastic transformation of cells. Apoptosis, programmed cell death, is known to occur in many malignant tumours but its relation to prognosis is not clear and remains to be determined. C-erbB-2 protooncogene encodes a transmembrane glycoprotein which has functional and molecular homology with epidermal growth factor receptor, shows a correlation with poor prognosis. In the present study, we investigated apoptotic index and the expression of cyclin D1 and c-erbB-2 in stage I endometrial carcinomas and their relation to the depth of myometriyal invasion, tumor grade and survival rate. STUDY DESIGN: A total of 60 stage I endometrial carcinomas, with a median follow-up of 29.6 months, were studied. Seven cases (11.6 %) were FIGO stage Ia, 30 (50 %) stage Ib and 23 (38.3 %) stage Ic. Parafin sections were stained with cyclin D1 and c-erbB-2 antibodies using the standard streptavidin-biotin-immunoperoxidase method. Apoptotic index was determined by the rate of apoptotic bodies-cells to vital tumour cells in hematoxylin-eosin stained sections. All these three parameters were correlated with the depth of miyometriyal invasion, grade and survival rate. RESULT (s): No significant relationship was found between cyclin D1, c-erbB-2 expression, apoptotic index and the depth of myometrial invasion, grade and survival rate (p>0.05). CONCLUSION (s): These findings suggest that cyclin D1 and c-erbB-2 expression and apoptotic index are not valuable predictors of myometrial invasion in endometrial carcinomas.

Siklinler prolifere olan hücrelerin hücre siklusuna girişi ve hücre siklusu progresyonunun kontrolü ile ilişkili bir grup proteindir. Siklin D1 proteininin G1 fazından S fazına geçişte rol oynadığı ve aşırı ekspresyonunun hücrelerde neoplastik transformasyona yol açtığı düşünülmektedir. Neoplastik büyüme proliferatif aktivite ve ölen hücrelerin oranı arasındaki dengeye bağlıdır. Birçok malign tümörde programlanmış hücre ölümü olan apopitozisin varlığı gösterilmiş ancak prognoz ile ilişkisi açıklık kazanmamıştır. C-erbB-2 protoonkogeni epidermal büyüme faktörü ile fonksiyonel ve moleküler benzerlik taşıyan bir transmembran glikoproteinini kodlar. Kimi tümörlerde c-erbB-2 ekspresyonunun kötü prognoz ile ilişkili olduğu saptanmıştır. Bu çalışmada evre I endometri-yum karsinomlarında apopitotik indeks, siklin D1 ve c-erbB-2 ekspresyonlarının miyometriyal invazyon derinliği, tümör derecesi ve sağkalım ile ilişkisi araştırılmıştır. MATERYAL ve METOD: Bu çalışmada endometriyum karsinomu tanısı almış 60 olgu yer almaktadır. Olguların 7'si (% 11.6) FIGO evre Ia, 30'u (% 50) evre Ib, 23'ü (% 38.3) evre Ic'dir. Ortalama izlem süresi 29.6 aydır. Parafin kesitlere standart streptavidin-biotin immünperoksidaz yöntemi kullanılarak siklin D1 ve c-erbB-2 antikorları ile immünhistokimyasal boyama yapılmıştır. Apopitotik indeks, apopitotik cisimlerin vital tümör hücrelerine oranı belirlenerek saptanmıştır. İstatistiksel yöntemler ile her üç parametre ile miyometriyal invazyon derinliği, tümör derecesi ve sağkalım arasındaki ilişki araştırılmıştır. BULGULAR: Siklin D1, c-erbB-2 ekspresyonları ve apopitotik indeks ile miyometriyal invazyon derinliği, tümör derecesi ve sağkalım arasında istatistiksel olarak anlamlı bir ilişki saptanmamıştır (p>0.05). SONUÇ: Bu çalışmada endometriyum karsinomlarında siklin D1 ve c-erbB-2 ekspresyonları ile apopitotik indeksin miyometriyal invazyonu belirlemede tek başına etkili olmadığı sonucuna varılmıştır.
