Serum cardiac troponin I as a predictor of heart diseases in long-term HTR users

Abstract
OBJECTIVE: To investigate the effects of oral conjugated estrogen plus medroxyprogesterone acetate in a continuously combined regimen and tibolone on serum cardiac troponin I levels in postmenopausal women. STUDY DESIGN: Seventy postmenopausal women, comprising long-term HRT user and nonusers, were included in this cohort study and were divided into three groups. First group consisted of 28 women using conjugated estrogen continuously combined with medroxyprogesterone acetate (CEE+MPA) for three years or longer, second group consisted of 18 postmenopausal women using tibolone more than three years and the third one as the control group consisted of 24 healthy women who are postmenopausal more than three years and not on any HRT regimens. Serum cardiac troponin I (cTnI) levels of all women were analyzed and body-mass indices (BMI) were calculated. RESULT(s): Mean cTnI levels were 0.012±0.02 ng/ml in first group (CEE+ MPA), 0.011±0.02 ng/ml in second group (tibolone), and 0.014±0.02 ng/ml in third group (control), respectively. Mean serum cardiac troponin levels were not statistically different between HRT groups and controls. No correlation between demographic variables, BMI and cTnI levels were found. CONCLUSION(s): These two HRT regimens seem not to have beneficial or adverse effects on serum cardiac troponin I levels and thus cardiovascular events.

AMAÇ: Menopoz sonrası kadınlarda sürekli medroksiprogesteronasetatla kombine edilmiş oral konjüge östrojen ve tibolonun serum kardiak troponın I üzerine etkisini araştırdık. MATERYAL ve METOD: Uzun dönem hormon replasman tedavisi (HRT) kullanan ve HRT almayan 70 kadın, bu ortak öz çalışmaya dahil edildi. Kadınlar üç gruba ayrıldı. Birinci grup üç yıl veya daha fazla medroksiprogesteron asetatla sürekli kombine edilmiş konjüge östrojen (CEE+ MPA) alan 28 kadından, ikinci grup üç yıl ve daha fazla süreyle tibolon kullanan 18 kadından ve üçüncü grup (kontrol) herhangi bir HRT kullanmayan, üç yıldan fazladır menopoz sonrası dönemde olan 24 kadından oluştu. Bütün kadınlarda serum kardiak troponın I (cTnI) çalışıldı ve vücut kitle indeksleri (VKI) hesaplandı. BULGULAR: Birinci grupta (CEE+MPA) ortalama cTnI: 0.012±0.02 ng/ml, 2. grupta (tibolon) 0.011±0.02 ng/ml olarak saptandı. Kontrol grubunda ise 0.014±0.02 ng/ml saptandı. Farklı HRT rejimleri arasında ve kontrol grubunda cTnI değerleri açısından anlamlı fark saptanmadı (p>0.1). Aynı zamanda ortalama cTnI iki farklı HRT grubunda da benzer bulundu. Demografik özellikler, BMI ve cTnI arasında bağlantı saptanmadı. SONUÇ: Bu iki HRT rejiminin cTnI aracılığı ile kardiovasküler sistem üzerine olumlu veya olumsuz etkisi yoktur.
