Maternal satisfaction and preference for delivery method in a subsequent pregnancy following cesarean section in healthcare staff

Abstract

OBJECTIVE: To compare the maternal outcomes in healthcare staff who delivered by elective cesarean section with others who delivered by cesarean section for obstetrical indications.STUDY DESIGN: We interviewed 144 of our healthcare staff (39 physicians, 88 nurses-midwifes, 17 healthcare personnel) employed in Edirne who completed their latest pregnancies by cesarean section. Cases were grouped whether they had an elective cesarean section (n=64) or had a cesarean section by an obstetrical indication (n=80). The questionnaire was constructed on the basis of reviews of published reports and existing questionnaires. We analyzed the data using "Minitab".RESULT(s): The cited reasons for requesting an elective cesarean section were fear of pain (n=20) or maternal anxiety due to possible trauma to the newborn during vaginal delivery (n=16), and both of them (n=10), fear of damage to the perineum (n=7), recommendation by her physician (n=6) and miscellaneous social factors (n=5). The reported satisfaction rate was 78% following the procedure. However, 53.4% told that they had wished to have a cesa-rean section in their earlier weeks' of gestation. Besides, 22.2% of the women who underwent cesarean section reported that they would prefer a vaginal delivery if they were to have another pregnancy. In women who had an elective cesarean section, the satisfaction rate was higher (57/64 vs. 56/80 women, p=0.01) and complication rate was lower (2/64 vs. 13/80 women, p=0.02) when compared to those who delivered by an obstetrical indication.CONCLUSION(s): The healthcare staff surveyed were less frequently in favor of preferring a vaginal delivery after cesarean section. Following an elective cesarean section, women were more satisfied and had lower complication rates.

AMAÇ: Sezaryenle doğum yapmış sağlık çalışanlarında, kendi isteğiyle elektif sezaryen olanların sonuçlarını endikasyon konularak sezaryen yapılanlarla karşılaştırabilmekti.MATERYAL ve METOD: Edirne ilinde halen çalışmakta olan ve son doğumunu sezaryenle gerçekleştirmiş toplam 144 sağlıkçıya (39 doktor, 88 ebe-hemşire, 17 yardımcı sağlık personeli) anket uygulandı. Olgular endikasyon yokken kendi isteğiyle elektif sezaryenle doğum yapmayı seçenler (n=64) ve endikasyon koyularak sezaryenle doğum yapanlar (n=80) olarak ikiye ayrıldı. Anket formu daha önce yapılan çalışmalarda incelenen konular ve konuyla ilişkisi olduğu düşünülen sorular temel alınarak oluşturuldu. İstatistiksel analizler "Minitab" programıyla gerçekleştirildi.BULGULAR: Olgularımızdan elektif sezaryen olanlar doğum ağrısı çekmekten (n=20) ya da doğum sırasında bebeğe travma olur diye korktuğu (n=16), hem doğum ağrısından hem de bebeğe travma olur diye korktuğu için (n=10), daha sonra gelişebilecek olası pelvik relaksasyon nedeniyle (n=7), doktor önerisiyle (n=6) veya sosyal nedenlerle (n=5) sezaryen tercih ettiklerini belirtti. Tüm olguların % 78'i sezaryen olmaktan memnun olduğunu, % 53.4'ü gebeliğin ilk dönemlerinde sezaryenle doğum yapmayı istemiş olduğunu, % 22.2'si de tekrar gebe kalırsa vajinal doğumu tercih edeceklerini söyledi. Elektif sezaryen olanlarda (57/64) hasta memnuniyeti endikasyon konularak sezaryen yapılanlara göre (56/80) daha yüksek (p=0.01) ve komplikasyon oranı daha düşük bulundu (2/64 kişi vs. 13/80 kişi, p=0.02).SONUÇ: Sağlık çalışanlarında sezaryen sonrası vajinal doğum tercihi azdır. Elektif sezaryen sonrası kişilerin memnuniyeti artmakta ve komplikasyon oranı da azalmaktadır.
