Prenatal diagnosis cytogenetic results of Akdeniz University Faculty of Medicine

Abstract
OBJECTIVE: Invasive methods prenatal diagnosis has been recognized as a safe reliable method for couples at high risk of giving birth to child with clinically significant chromosomal abnormality or other genetic diagnosis. Chorionic villus sampling (CVS), amniocentesis (AS) and cordocentesis (KC) have been used as prenatal diagnosis techniques since 30 years. AS is performed 15 weeks gestation and is the most widely used invasive technique. CS is performed after 18 to 20 weeks of pregnancy and CVS may be carried out after 11 weeks.STUDY DESIGN: The most common indications are a raised risk of Down's syndrome and other trisomies, advanced maternal age tripte testing pathological ultrasonographic findings. Prenatal karyotypes were undertaken in 2084 amniotic fluid samples, 122 CVS, and 199 CS. The frequency of fetal abnormal karyotypes were 2.95% (71/2405 cases).RESULT(s): Among the fetal abnormal karyotypes, the frequency of numerical chromosome abnormalities and structural chromosome abnormalities were 1.87 % (45/71 cases) and 1.08 % (26/71 cases), respectively. In the numerical abnormalities, the frequency of trisomy 21 was by far the highest (21/45 cases), followed by sex chromosome aneuploidies in 15 cases. In the structural chromosomal rearrangements, 2 cases had familial inversions chromosome 17 and Y. Chromosomal deletion in one case, duplication one case and extra marker chromosome in two cases were detected. The remaining 19 cases had translocation, 11 of them had reciprocal translocations and 8 cases had Robertsonian translocations. Among them, 13 cases had balanced translocation and 6 had unbalanced. Out of 19 cases with translocation carriers, sixteen cases were inherited from one of the parents and three cases were de novo, genetic counceling was given for families.

AMAÇ: İnvaziv yöntemlerle prenatal tanı, klinik olarak önemli kromozom anomalisine ve/veya diğer genetik hastalıklara sahip, çocuk doğurma riski yüksek olan çiftlerde uygulanan güvenilir bir metoddur. Yaklaşık 30 yıldır uygulanan prenatal tanı teknikleri, amniyosentez (AS), kordosentez (KS) ve koryonik villüs aspirasyonu (CVS) yöntemleridir. AS, hamileliğin 15. haftasından itibaren uygulanan en yaygın invaziv tekniktir. KS, gebeliğin 18. ve 20. haftalarından sonra, CVS ise 11. haftadan sonra uygulanmaktadır.MATERYAL ve METOD: Down sendromu ve diğer trizomik sendromlar için en yaygın endikasyonlar, ileri anne yaşı, üçlü tarama testi ve patolojik ultrasonografik bulgulardır. Bu tekniklerin kullanılması ile gerçekleştirilen prenatal tanı ile ilgili sitogenetik analizler uygulanmış, 2084 AS, 122 CVS ve 199 KS örneklerine karyotip analizleri yapılmıştır.BULGULAR: Anormal fetal karyotiplerinin sıklığı % 2.95 (71/2.405 olgu) olarak bulunmuştur. Sayısal kromozom bozukluklarının sıklığı % 1.87 (45/71 anormal karyotipli olgu), yapısal kromozom bozuklukların sıklığı ise % 1.08 (26/71 anormal karyotipli olgu) olarak belirlenmiştir. Sayısal kromozom anomalilerinden Trizomi 21 en sık gözlenirken (21/45 olgu), 15 olgu ile seks kromozom anöploidileri ikinci sırayı almıştır. Yapısal kromozomların yeniden düzenlemeleri içerisinde, 2 olguda ailesel olarak kalıtım gösteren 17 ve Y kromozomlarının inversiyonları gözlenmiştir. Bir olguda delesyon, bir olguda duplikasyon ve 2 olguda ekstra marker kromozom belirlenmiştir. 19 olguda translokasyon gözlenmiş, bunların 11'inde resiprokal ve 8'inde ise Robertsonian translakosyon belirlenmiş, 13 olgunun dengeli ve 6 olgunun dengesiz translokasyon taşıyıcısı olduğu saptanmıştır. On altı olguda translokasyon, ebeveynlerin birinden kalıtım göstermiş, 3'ünde ise de novo olarak ortaya çıkmış, ailelere genetik danışma verilmiştir.
