Transobturator tape: a new method in treatment of stress urinary incontinence

Abstract

OBJECTIVE: To determine the short term success and complications rates of the transobturator tape (TOT) in treatment of female stress urinary incontinence (SUI).STUDY DESING: The study was designed as a prospective clinical trial. The patients who had attended to Baskent University Faculty of Medicine Dept. of OB-GYN through March 2004-May 2004 with complaint of SUI were evaluated. Total of 16 patients who had established diagnosis of SUI with simple cystometry, Q-tip and cough stress test were underwent TOT procedure. The characteristic features of patients, the duration of operation, intraoperative and postoperative complications were noted. The patients were reevaluated with Q-tip and cough stress test after 1, 3 and 6 weeks after the operation.RESULT(s): There were 16 patients who underwent TOT surgery (outside-in) because of stress urinary incontinence (SUI). The mean duration of the operation was 15.3±3.7 (min). The mean durations of the urinary catheter and the hospitalization were 20.1±5.3 (hr) and 24.2±2.8 (hr) respectively. There was no intraoperative complication in all patients. In one patient (1/16; 6.25 %), the cystoscopy was performed intraoperatively due to a suspicion of bladder injury, but no bladder or urethral injury was noticed in cystoscopic examination. The spontaneous urination after the removal of the urinary catheter was obtained and all patients were discharged from hospital without any problem. There was only one patient (1/16; 6.25 %) with cough test positive in examination performed 1 week after the surgery. Adjustment of the hammock was performed at the office with local anesthesia. The mentioned patient and others was all cough test negative in examinations performed 3 and 6 weeks after the operation. There was no postoperative voiding dysfunction in all patients. In one patient (1/16; 6.25 %), infection and abscess formation in one of the 5 mm incisions was encountered. Fortunately she was symptom-free after the abscess drainage and treatment with appropriate antibiotics.CONCLUSION: Transobturator Tape (TOT) may be used safely in treatment of stress urinary incontinence (SUI) with lower intraoperative and postoperative complication rates compared to classic slings. However, larger prospective randomized studies were needed to confirm these results.

AMAÇ: Stres üriner inkontinans tedavisinde (SÜİ) Transobtüratör Tape (TOT) operasyonunun kısa dönem sonuçlarının ve komplikasyonlarının belirlenmesi.MATERYAL ve METOD: Çalışma prospektif klinik bir çalışma olarak planlandı. Başkent Üniversitesi Kadın Hastalıkları ve Doğum bölümüne Mart 2004-Mayıs 2004 tarihleri arasında SÜİ şikâyeti ile başvuran hastalar değerlendirmeye alındı. Basit sistometri, Q-tip ve öksürük testleri ile SÜİ tespit edilen 16 hastaya TOT uygulandı. Hastaların demografik özellikleri, operasyon süresi, intraoperatif ve postoperatif komplikasyonları not edildi. Hastalar işlem sonrası 1. hafta, 3. hafta ve 6. haftalarda kontrole çağrıldı. Kontrol sırasında mesane tam dolu olarak öksürük testi ve Q tip testi yapıldı.BULGULAR: Toplam 16 hastaya SÜİ nedeni ile TOT (dıştan içe) yapıldı. Ortalama operasyon süresi 15.3±3.7 dakikaydı. Operasyon sonrası ortalama sonda kalış süresi 20.1±5.3 saat olup, ortalama hastanede kalış süresi 24.2±2.8 saat olarak tespit edildi. İntraoperatif komplikasyon hiçbir olguda izlenmedi. Sadece bir olguda (1/16; % 6.25) mesane perforasyonu şüphesinden dolayı intraoperatif sistoskopi yapıldı. Sistoskopide herhangi bir mesane ya da üretral zedelenme izlenmedi. Postoperatif sondanın çekilmesi ile birlikte tüm hastalar spontan idrarını yaparak sorunsuz olarak taburcu edildi. Birinci hafta kontrolde olgulardan sadece 1 tanesine (1/16; % 6.25) ayakta öksürük testi pozitif olması nedeni ile ofis ortamında lokal anestezi kullanılarak hamak ayarlaması yapıldı. Bu hastanın ve diğerlerinin 3. ve 6. hafta kontrollerinde öksürük testi negatif olarak tespit edildi. Kontrollerde hiçbir hastada üriner disfonksiyon izlenmedi. Bir hastada (1/16; % 6.25) 5 mm'lik insizyonların birisinde enfeksiyon ve apse gelişti. Apse drenajı ve antibiyotik tedavisi ile sorunsuz olarak iyileşti.SONUÇ: Transobtüratör Tape (TOT) operasyonu Stres Üriner İnkontinans (SÜİ) tedavisinde klasik askılama yöntemlerine göre daha az intraoperatif ve postoperatif komplikasyonlar ile kullanılabilir. Fakat, bu sonucun desteklenmesi için daha geniş prospektif randomize çalışmalara ihtiyaç duyulmaktadır.
