Pelvic splenosis: Case report and literature review

Abstract
Splenosis is the heterotopic autotransplantation of splenic tissue that usually follows traumatic splenectomy. A 40-year-old woman presenting with low abdominal discomfort, and pelvic mass was misdiagnosed as malignancy. She had post-traumatic splenic rupture and laparotomy history. Pelvic splenosis was diagnosed at laparotomy. Because splenosis is not a pathologic process but may actually be beneficial to the patient, it is important to recognize splenosis and not confuse it with endometriosis, malignancy, or hemangioma. As splenosis is usually asymptomatic and may partially compensate for the asplenic state, it is recommended that asymptomatic splenosis not to be resected when encountered at surgery. When the splenosis is diagnosed incidentally in a symptomatic patient, the complete surgery removal is not indicated. However this surgery is recommended when the abdominal pain or the diagnosis is uncertain. The authors recommend that the nodules of splenosis are probably a defense against infections affecting "splenic" patients. Consequently, the removal of only symptomatic ectopic splenic tissue is indicated. Because our patient was symptomatic and malignacy was uncertain, this pelvic mass was extirpated and abdominal hysterectomy and bilateral salpingooopherectomy was done with the diagnosis of uterine fibroid

Splenozis splenik dokunun heterotopik ototransplantasyonu olup, genellikle travmatik dalak rüptürü, splenektomi veya cerrahi sonrası gelişir. 40 yaşında, G6P4, beş yıl önce travma sonrası splenektomi öyküsü olan ve kasık ağrısı şikayeti ile başvuran hastada pelvik kitle, malignite ön tanısı konularak operasyona hazırlandı. Operasyon sonrasında pelvik splenosis tanısı patolojik olarak konuldu. Splenozis, asemptomatik olduğu gibi, bazen de pelvik ağrı, disparoni, dismenore ve/veya kitle şikayetleri ile ortaya çıkabilir. Splenozis patolojik bir işlev olmadığından ve hastanın yararına olduğundan bu lezyonu tanımak ve endometriozis, malignite ve hemanjiyom ile karıştırmamak önemlidir. Splenozis genelde asemptomatik olduğundan ve asplenik durumu kısmen kompanse ettiğinden cerrahi sırasında karşılaşıldığında asemptomatik splenozisin çıkarılmaması tavsiye edilir. Semptomatik hastalarda teşhis edildiğinde tümüyle cerrahi olarak çıkarılması endike değildir. Cerrahi ağrının nedeni başka bir olaya bağlanamadığında veya tanı kesin konulamadığında tavsiye edilmektedir. Hastada malignite ayırt edilemediğinden ve semptomatik olduğundan kitle ekstirpe edilerek frozen-section için gönderildi. Ayrıca miyoma uteri nedeni ile TAH+BSO yapıldı.
