Correlation of patient history and urodynamic findings in evaluation of the type of urinary incontinence

Abstract
OBJECTIVE: In the differantial diagnosis of incontinence, we compare the history and urodynamic testing.STUDY DESIGN: Between January 2003-April 2004, 304 women with a primary complaint of urinary incontinence were evaluated in our urogynecology clinic. The patients who had systemic and metabolic disease, had positive urinary culture, had prior gynecologic operation were excluded. Cases were grouped according to the history as stress, urge and mixed incontinence. Afterwards incontinence was evaluated with urodynamic testing. Sensitivity, specifity, positive predictive value, negatif predictive value, total diagnosis value were calculated for the history in the differential diagnosis.RESULTS: Of 304 cases, 97 was diagnosed as stress, 49 as urge, 158 as mixed incontinence according to the patients history. After urodynamic testing, sensitivity, specifity, positive predictive value, negative predictive value, total diagnosis value of stress urinary incontinence history were calculated as 43 %, 75.4 %, 53.6 %, 66.7 %, 62.5 % respectively. Sensitivity, specifity, positive predictive value, negative predictive value, total diagnosis value of urge urinary incontinence history were calculated as 40.4 %, 88.9 %, 42.9 %, 87.8 %, 80.6 % respectively. Sensitivity, specifity, positive predictive value, negative predictive value, total diagnosis value of mixed urinary incontinence history were calculated as 68.9 %, 51 %, 19.6 %, 90.4 %, 53.6 % respectively.CONCLUSION: Patient history alone is not accurate tool in the diagnosis of the urinary incontinence type. History and urodynamic testing should be combined.

AMAÇ: Anamnezin inkontinans tipini belirlemede, anamneze dayalı tanı ile ürodinamiye dayalı tanıların uyumluluğunu ortaya koymak.MATERYAL ve METOD: Ocak 2003-Nisan 2004 tarihleri arasında idrar kaçırma şikâyeti ile ürojinekoloji kliniğine başvuran 304 hasta değerlendirildi. Herhangi bir sistemik ve metabolik hastalığı olan, idrar kültüründe enfeksiyon saptanan, daha önce jinekolojik bir operasyon geçiren hastalar çalışmaya alınmadı. Olgular önce anamneze göre stres, sıkışma ve mikst inkontinans olarak gruplandırıldı. Daha sonra ürodinamik inceleme ile inkontinans tipleri saptandı. Anamnezin inkontinans tipini belirlemedeki sensitivitesi, spesifitesi pozitif prediktif değeri, negatif prediktif değeri ve toplam tanı değeri hesaplandı.BULGULAR: 304 olgu sadece anamneze göre değerlendirildiğinde 97 olgu stres inkotinans, 49 olgu sıkışma inkontinans, 158 olgu ise, miks inkotinans tanısı aldı. Tüm olgulara ürodinamik inceleme yapıldığında ise, anamnezin stres inkontinans tanısında sensitivitesi % 43, spesifitesi % 75.4, Pozitif belirleyicilik değeri (PBD) % 53.6, Negatif belirleyicilik değeri (NBD) % 66.7, toplam tanı değeri % 62,5 olarak hesaplandı. Anamnezin sıkışma inkontinans tanısında sensitivitesi % 40.4, spesifitesi % 88.9, Pozitif prediktif değeri % 42.9, Negatif belirleyicilik değeri % 87.8, toplam tanı değeri % 80.6 olarak hesaplandı. Anamnezin miks inkontinans tanısında sensitivitesi % 68.9, spesifitesi % 51, Pozitif belirleyicilik değeri % 19.6, Negatif belirleyicilik değer % 90.4, toplam tanı değeri ise, % 53.6 olarak hesaplandı.SONUÇ: Anamnez inkontinans tipini saptamada tek başına yeterli tanı değerine sahip değildir. İnkontinans tipini saptamada anamnez ve ürodinami verileri birlikte değerlendirilmelidir.
