Transobturator tape: One year experience

Abstract
OBJECTIVE: To determine the one year follow-up success and complications rates of the transobturator tape (TOT) operation in the treatment of female stress urinary incontinence (SUI).STUDY DESIGN: A total of 32 patients who underwent TOT procedure for urinary stress incontinence in Baskent University Department of Obstetrics and Gynecology through March 2004-January 2005 were included into the study. The basal characteristics, the duration of the operation, the presence of intraoperative and postoperative complications were noted. The evaluation of the patients approximately after one year of the operation was performed.RESULTS: There were 32 patients who underwent TOT surgery (outside-in) because of stress urinary incontinence (SUI). The mean duration of the operation was 13.3±2.9 (min). The mean durations of the urinary catheter and the hospitalization were 19.3± 7.2 (hr) and 28.3±3.2 (hr), respectively. There was no intraoperative complication in all patients. After the postoperative withdrawal of the urinary catheter, no patients have voiding disfunction. In 3 patients (3/32; 9.4 %), infection of the 5 mm incisions was encountered. In 2 of these patients, the withdrawal of the TOT hammock should be necessitated even use of antibiotics and abscess drainage. However, others uneventfully treated by use of antibiotics only. In four of the remaining 30 patients (4/30, % 13.3) the cough test was positive and all of 4 had symptomatic stress urinary inconstance. The one year success of the TOT operation was determined as 86.7 %.CONCLUSION: Transobturator Tape (TOT) can be used safely and successfully in the treatment of stress urinary incontinence (SUI). However, postoperative infection should not be overestimated.

AMAÇ: Stres üriner inkontinans tedavisinde (SÜİ) Transobtüratör Tape (TOT) operasyonunun bir yıllık sonuçlarının belirlenmesi.MATERYAL ve METOD: Başkent Üniversitesi Kadın Hastalıkları ve Doğum bölümünde Mart 2004-Ocak 2005 tarihleri arasında SÜİ şikâyeti ile TOT uygulanmış 32 hasta çalışmaya dâhil edildi. Hastaların bazal karakteristik özellikleri, operasyon süresi, intraoperatif ve postoperatif komplikasyonları not edildi. Bir yıl sonraki kontrolde Q-tip ve öksürük testleri ile hastalar tekrar değerlendirildi.BULGULAR: Toplam 32 hastaya SÜİ nedeni ile TOT (dıştan içe) yapıldı. Ortalama operasyon süresi 13.3±2.9 dakika idi. Operasyon sonrası ortalama sonda kalış süresi 19.3±7.2 saat olup, ortalama hastanede kalış süresi 28.3±3.2 saat olarak tespit edildi. İntraoperatif komplikasyon hiçbir olguda izlenmedi. Postoperatif sondanın çekilmesi ile birlikte tüm hastalar kendiliğinden idrarını yaparak, sorunsuz olarak taburcu edildi. Postoperatif dönemde 3 olguda 5 mm'lik insizyon yerinde enfeksiyon gelişti. İki olguda drenaj ve antibiyotik tedavisine cevap izlenmediği için TOT hamağının çekilmesi gerekti. Bir yıllık kontrolde olgulardan 4 tanesinde (4/30; % 13.3) ayakta öksürük ve Q-tip testi pozitif bulundu. Bu 4 olgunun semptomatik olarak da SÜİ şikâyeti mevcuttu. Transobtüratör Tape'in 1 yıllık başarı oranı % 86.7 olarak tespit edildi.SONUÇ: Transobtüratör Tape operasyonu SÜİ tedavisinde güvenli ve başarılı olarak kullanılabilir. Fakat postoperatif enfeksiyon gelişmesinin engellenmesi gereklidir.
