Doppler flow changes in oxytocin versus prostaglandin E1-induced active labor

Abstract

OBJECTIVE: The aim of this prospective study is to compare the effect of oxytocin versus misoprostol on Doppler resistance indices of the uterine and umbilical artery during labor induction.STUDY DESIGN: Seventy pregnant women with indications for induction of labor assigned to receive either intravenous oxytocin or misoprostol 50 µg per vagina every 6 h as needed. Doppler velocimetry of umbilical and bilateral uterine arteries were performed before and three hours after administration of oxytocin or misoprostol and in active phase of labor. In these arteries, pulsatility index (PI), resistance index (RI) and systolic/diastolic ratios (S/D) were measured.RESULTS: Demographic data of the two groups were comparable. In the latent and active phase of labor, PI, RI and S/D ratios in the umbilical artery were not change. These indices in evaluation of the uterine arteries were significantly increased in latent phase compared with those measured before labor induction with oxytocin or misoprostol. There were no significant changes of PI, RI and S/D ratios of all arteries between latent and active phase of labor. The measured values of Doppler parameters of uterine arteries in misoprostol group were significantly higher than those in oxytocin group during the active phase, but they were similar in evaluation of the umbilical artery. Significant positive linear correlation was observed between high S/D ratio in the umbilical artery before induction and the increased risk of caesarean section (r=0.282, p< 0.05).CONCLUSION: Uteroplacental resistance increases in labor induction with oxytocin or misoprostol, but umbilical circulation is not affected. The high degree of S/D ratio in the umbilical artery before induction may be indicating the increased risk of caesarean section.

AMAÇ: Bu prospektif çalışmanın amacı, oksitosin ve misoprostolle yapılan doğum indüksiyonunda, uterin ve umblikal arter doppler akım direnci belirteçlerini karşılaştırmaktır.MATERYAL ve METOD: Doğum indüksiyonu için endikasyon koyulan 70 gebe kadına intravenöz oksitosin veya gerektikçe 6 saatte bir 50 µg vajinal olarak misoprostol verildi. Uterin ve umblikal arterlerin doppler değerlendirmesi indüksiyon öncesi, indüksiyondan 3 saat sonra (latent faz) ve aktif fazlarda yapıldı. Bu arterlerde pulsatilite indeksi (PI), rezistans indeksi (RI) ve sistol/diastol (S/D) oranı ölçüldü.BULGULAR: İki grubun demografik özellikleri benzerdi. Uterin arterlerin incelemesinde, doğumun latent ve aktif fazlarında umblikal arter PI, RI ve S/D oranları değişmedi. Bu belirteçler latent fazda, oksitosin ve misoprostol ile doğum indüksiyonuna başlamadan önceki ölçüme göre belirgin artış gösterdi. Doğumun latent ve aktif fazları arasında tüm arterlerde PI, RI ve S/D oranı açısından önemli bir değişiklik görülmedi. Aktif fazda uterin arterde ölçülen doppler parametreleri misoprostol grubunda oksitosin grubunda ölçülenlere göre önemli derecede yüksekti, fakat umblikal arter ölçümleri benzerdi. İndüksiyon öncesi dönemde umblikal arterde tespit edilen yüksek S/D oranı ile artmış sezaryen riski arasında pozitif lineer ilişki görüldü (r=0.282, p< 0.05).SONUÇ: Oksitosin veya misoprostol ile yapılan doğum indüksiyonu uteroplasental direnç artışına yol açar, fakat umblikal dolaşım bundan etkilenmez. İndüksiyon öncesi tespit edilen yüksek S/D oranı artmış sezaryen riskini gösterebilir.
